Long COVID Awareness #4 : wat is er nu bekend over post-COVID? [ Inclusief terugblik op 20 jaar kennis over het Post-SARS Syndroom ]

Wat is Long COVID of Post-COVID Syndroom?
Long COVID is de gebruikelijke term voor Post-Acute SARS of Post-COVID-Syndroom. Het betekent dat klachten aanhouden nádat iemand van COVID herstelt lijkt te zijn. Long COVID is een verzamelterm. Verschillende klachten kunnen onder Long COVID vallen en Long COVID heeft verschillende oorzaken. Iemand kan bijvoorbeeld voor lange tijd ontstekingsreacties hebben, vergeetachtig zijn of longproblemen hebben. Die longproblemen kunnen worden veroorzaakt doordat het virus langere tijd in het lichaam aanwezig blijft, of doordat het virus bloedstollingsziekten (trombose) veroorzaakt. Trombose, endotheliumschade (schade aan het weefsel waarmee de vaten zijn bekleed), fibrose en aanhoudende activering van ontstekingsfactoren zijn kenmerkend voor zowel COVID als Long COVID.

Long COVID is niet nieuw. Sterker nog, het verschijnsel is al 20 jaar in de wereld, al is de term pas in 2020 geïntroduceerd. De voorloper van COVID-19, de SARS-CoV-1-epidemie, duurde van 2003 tot 2005. Na de SARS-1-epidemie werden soortgelijke klachten als bij Long COVID gemeld.
Sinds de eerste SARS-epidemie van 2003-2005 is het Post-Acute SARS Syndroom, de equivalent van Long COVID (Post-COVID), geregeld geëvalueerd.

Het is reeds 20 jaar bekend dat Long COVID (= Post-Acute SARS) aanhoudende trombose, fibrose en endotheliopathie met MOF (multi-orgaanfalen) tot gevolg is.

Evenals begin 2020 wordt bevestigd dat leefstijl, (over)gewicht/BMI en medische voorgeschiedenis niet van invloed zijn op de ernst van COVID en de ontwikkeling van Long COVID.

1.1. Algemeen bekende klachten bij Long COVID (of Post-Acute SARS Syndroom):
- Een verslechterde longfunctie;
- Endotheliopathie en schade aan de glycocalyx;
- Mitochondriale dysfunctie (daaronder begrepen een verstoord celmetabolisme);
- Ontstekingen aan het hart;
- Cognitieve problemen;
- Neurologische problemen;
- Bloedstollingsziekten (trombose, falende trombolyse);
- Fibrotische orgaanschade (falende fibrinolyse);
- Oorsuizen, misselijkheid, aanhoudende keelpijn, spierzwakte, nierproblemen, lage bloeddruk, zwakke pols, huiduitslag, donkere urine

1.2. Kenmerken van Long COVID:
- Verslechterde longfunctie (gemeten aan de hand van DLCO (koolmonoxidediffusie), TLC (totale longcapaciteit) en prestatie op de 6MWT (6-minuten-looptest));
- Immuungerelateerde verstoring van het autonome systeem, met orthostatische intolerantie (POTS) tot gevolg. POTS is Posturale Orthostatische Tachycardie, een storing in het autonoom systeem die acute veranderingen teweegbrengt in de bloeddruk, polsslag en tolerantie voor positieverandering;
- Aanwezigheid van LGE (late gadolinium enhancement, een marker voor fibrose door ontsteking van het hartzakje) en een verhoging van troponine;
- Longfibrose;
- MIS-A: aanhoudende ontstekingsreacties van het lichaam;
- Een aanhoudend lager niveau van witte bloedcellen

1.3. Case study: trombose in het hart van een 21-jarige met Post-COVID
In een recente rapportage over een 21-jarige zonder medische voorgeschiedenis wordt melding gemaakt van trombose in de onderste holle ader en wanden van de kamers van het hart. In de vier maanden voorafgaand aan de rapportage was het CRP significant verhoogd. SARS-CoV-2 veroorzaakt niet alleen trombose door hypercoagulatie en overactivering van het endothelium; microtrombi ("microclots") zijn antigenen die een autoimmuunreactie kunnen veroorzaken (Post-Acute COVID syndrome (PACS)  right atrioventricular and vena cava thrombus on top of a myxoma: A Case Report, Journal of Cardiothoracic Surgery (2022) 17:261).

De vorming van microtrombi wordt bovendien versterkt, doordat SARS fibrinolyse en trombolyse (het afbreken van bloedstolsels) tegengaat (Mechanisms of SARS Coronavirus-Induced Acute Lung Injury, American Society for Microbiology, July/August 2013, Volume 4 Issue 4).

2. Wat zijn de mogelijke oorzaken van Long COVID?

De Long COVID-Pyramide
Aan Long COVID liggen drie mechanismen ten grondslag:

1. Schade aan het endothelium & glycocalyx en activering van endotheelcellen;
2. Trombose en hypercoagulatie;
3. Aanhoudende auto-immuunreactie


2.1. Aanhoudende inflammatie van een specifiek fenotype
Bij SARS-CoV-1 werd 60 dagen na herstel, aanhoudende ontstekingsactiviteit waargenomen. Macrofagen, NK-cellen (Natural Killer Cells), CD8+-T-cellen en ontstekingsfactoren (IL-6 en TNF-alfa) bleven verhoogd aanwezig, terwijl in de macrofagen in de longblaasjes persistente virusdeeltjes werden aangetroffen. Opmerkelijk genoeg was de activiteit van neutrofielen in reactie op IL-8 ondermaats (Persistence of lung inflammation and lung cytokines with high-resolution CT during recovery from SARS, Respiratory Research 2005; 6(1): 42).

Een analyse van de casus van 47.910 volwassen Long COVID-patiënten weergeeft een opmerkelijk beeld. Er is in het algemeen weinig bewijs voor systemische ontstekingsreacties in Long COVID; de ontstekingsmarkers IL-6, TNF-alfa en IL-2 en de marker CRP zijn niet significant verhoogd (More than 50 long-term effects of COVID: a systematic review and meta-analysis, Sci Rep. 2021;11(1):16144).

Opmerkelijk is de medio 2022 gepubliceerde bevinding dat een subgroep van Long COVID-patiënten IL-8 en IFNγ-deficiënties heeft. Het niveau van IL-2, IL-6, IL-17 (inclusief Th17), IL-13 en IL-4 is in deze groep ook sterk gereduceerd. Dat Long COVID-patiënten een tekort aan IL-8 hebben, kan worden verklaard door een eerdere overactivering, gevolgd door uitputting. De uitputting van IL-8 heeft tot gevolg dat de wondheling van het weefsel in de organen in een later stadium wordt verhinderd. Aangezien infiltraten van door SARS geïnfiltreerde macrofagen voor schade aan het longweefsel zorgen, verklaart een IL-8-tekort de suboptimale genezing (Cytokine Deficiencies in Patients With Long COVID, Journal of Clinical and Cellular Immunology Vol 13, Issue 6). Deze bevinding lijkt echter tegengesteld aan bevindingen dat NETs en IL-8 voor aanhoudende fibrose en trombose bij Long COVID-patiënten zorgen.

2.2. SARS-CoV-2/COVID-patiënten kunnen anti-ACE2-antilichamen ontwikkelen

Eén van de belangrijkste receptoren voor het virus SARS-CoV-2 is de receptor voor ACE2, een eiwit dat betrokken is bij de bloeddrukregulering. Als een patiënt immuniteit tegen het bindingsdomein van COVID (RBD) ontwikkelt, kan het lichaam ook een antilichaam ontwikkelen tegen het lichaamseigen ACE2-eiwit. Dit heeft ontregeling van de bloeddruk en overactiviteit van het immuunsysteem tot gevolg (Development of ACE2 autoantibodies after SARS-CoV-2 infection, PLoS One 2021; 16(9) ).

2.3. Microtrombose en endotheliopathie: een veranderde VWF:ADAMTS-13-ratio en verhoging van Factor VIII
In een recente evaluatie zijn 330 post-COVID-patiënten getest op de 6-minute-walking-test, een test om het uithoudingsvermogen te beoordelen.
26% van de geteste personen met post-COVID had een verhoging van Factor VIII. Van alle patiënten met beperkingen in het normale uithoudingsvermogen, had maar liefst 55% een verhoging van de VWF:ADAMTS13-ratio (Impaired exercise capacity in post-COVID-19-syndrome: the role of the VWF-ADAMTS13-ratio, Thrombosis and Haemostasis Vol. 6, Issue 13).

De VWF, Von Willebrand Factor, is een trombotische factor, betrokken bij de bloedstolling. ADAMTS-13 of ADAMTS13 is een anti-trombotische factor, een soort "schaar" die de VWF in bedwang dient te houden. Bij infectie met het SARS-Coronavirus heeft de VWF de neiging om de overhand te krijgen. Dit leidt tot meer aanmaak van stollingsfactoren, onderdrukking van fibrinolyse (de normale verwijdering van stolsels), ontregeling van endotheelcellen en een staat van hypercoagulatie.

Net als bij COVID, zijn de vorming van microtrombi en ontregeling van het endothelium (het weefsel waarmee de bloedvaten en organen zijn bekleed) kenmerkend voor Long COVID. De VWF:ADAMTS13-ratio zou standaard moeten worden gemeten bij Long COVID. Ook is orgaanschade bij Long COVID fysiek vast te stellen door MRI, zo blijkt uit het project-#COVERSCAN (Multiorgan Impairment in low-risk individuals with Post-COVID-19-Syndrome: a prospective, community-based study, BMJ Open 2021; 11(3)).

De MRI en CT zijn echter niet altijd toereikend om te onderscheiden tussen microtrombose en lokale ontstekingen bij Long COVID-patiënten. Aanvullend onderzoek is nodig: een biopt en V/Q SPECT-scan kunnen onontdekte microtrombi in beeld brengen.

- 6-MWT uitvoeren
- D-dimeer en FVIII-serumniveau meten
- VWF:ADAMTS13-ratio meten
- MRI toepassen op orgaanschade en microtrombi
- Om te onderscheiden tussen microtrombi en lokale ontstekingen, biopt afnemen en/of V/Q SPECT uitvoeren

2.4. De Spike-proteïne van SARS vernietigt mitochondria en veroorzaakt DNA-schade in hartweefsel
Het is algemeen erkend dat COVID een verhoogd risico geeft op hartinfarcten, myocarditis, hartfalen, schade aan het hartweefsel, trombose en ritmestoornissen ("Long-term cardiovascular outcomes of COVID", Nature Med 2022;28(3)).

Het Spike-proteïne en het NSP13-proteïne van SARS-CoV-2 houden herstel van door het virus beschadigd DNA tegen. Door de mitochondria (het deel van de cel dat verantwoordelijk is voor de stofwisseling) te manipuleren, verschaft het virus zich een weg door de cellen van de gastheer. Dysfunctionele mitochondria houden ontstekingen aan het hartweefsel in stand; bovendien veroorzaken vrijgekomen deeltjes van de door COVID gekaapte mitochondria de voortijdige dood van hartcellen.

In COVID-patiënten met trombocytopenie (een tekort aan bloedplaatjes) en coagulopathie (een stollingsstoornis) worden regelmatig anti-cardiolipin-antilichamen tégen de mitochondria gevormd, waardoor schade aan het hartweefsel optreedt ("Transcriptomic profiling of cardiac tissues from SARS-CoV-2 patients identifies DNA damage", Immunology 2022;1-17).

2.5  Mestcel-activeringssyndroom (MCAS) als oorzaak van Long COVID
Mestcellen of mastocyten zijn betrokken bij de regulering van het immuunsysteem. Ze worden gevormd in het beenmerg. Om te kunnen rijpen, dienen ze zich vast te zetten op weefsels. Daar drukken ze antigenen FcεRI (de IgE-receptor) en Kit (een stamcelreceptor) uit. Mestcellen zijn betrokken bij allergische (ontstekings)reacties. De functie van de mestcel is meerzijdig: de granulen van mestcellen (blaasjes) bevatten zowel antistollingsfactoren/anticoagulanten zoals tPA en heparine, als ontstekingsfactoren/endotheliumactivatoren zoals TNF en histamine.

Mastocyten reguleren macrofagen, monocyten, dendrieten (de uitlopers), B-cellen, T-cellen en Natural Killer-cellen (NKs). Mestcellen zijn de belangrijkste bron voor de aanmaak van cytokinen in het vaatweefsel van de longen (Mast cells activated by SARS-CoV-2 releases Histamine which increases IL-1 levels causing cytokine storm and inflammatory reaction in COVID, Journal of Biological Regulators and Homeostatic Agents 2020, Vol. 34, Issue 5).

Activering van epitheelcellen in de longen en fibroblasten door SARS-CoV-2 zet de reactie met mestcellen en ontstekingsfactoren in gang. Beschadigde endotheelcellen en epitheelcellen en cytokinen stimuleren mestcellen. Mestcellen detecteren het virus via Toll-like receptor 3 (TLR-3). Bovendien beschikken mestcellen in de longen over de ACE2-receptor, één van de belangrijkste bindingsdomeinen van SARS-CoV-2 (COVID hyperinflammation and Post-COVID illness may be rooted in mast cell activation syndrome, International Journal of Infectious Diseases 100 (2020) 327-332). 

In de acute fase stimuleert SARS-CoV-2 de productie van IL-1 via mestcellen. IL-1 is toxisch voor het vaatweefsel. In het longweefsel (interstitieel en in de longblaasjes) afkomstig van autopsies van COVID-patiënten worden grote hoeveelheden chymase-positieve mestcellen met degranulatie in combinatie met oedeem, bloedingen en necrose van het longweefsel aangetroffen. Chymase is een ontstekingsfactor die door mestcellen wordt vrijgelaten. Chymase activeert TGF-beta en MMP-9, een proces dat resulteert in fibrose en hypercoagulatie in COVID (Role of Mast Cells in the Pathogenesis of Severe Lung Damage in COVID-19 Patients, Respiratory Research (2022) 23:371). Bovendien worden mestcellen door IL-33 en Substantie P gezamenlijk geactiveerd, om de aanmaak van VEGFs (Endothelial Growth Factors), IL-1beta en TGF te verhogen.

Door stimulatie van mestcellen komen ontstekingsfactoren zoals histamine, Prostaglandinen (PGs), tryptase en chymase, leukotrienen, heparinen, TNF-α, Il-1β, groeifactoren en Platelet Activating Factor (PAF) vrij. SARS-CoV-2 activeert mestcellen via de GPRX2-receptor, terwijl de afgifte van substantie P en neurotensine gelijktijdig wordt bevorderd (Pathophysiology of Post-COVID Syndromes: a new perspective, Virology Journal (2022) 19:158).

Substantie P en neurotensine zorgen voor doorlaatbaarheid van het vaatweefsel én de Bloed-Hersenbarriere (BBB) en voor ontsteking van het longweefsel. Het zijn bradykinine, neurolysine, substantie P en neurotensine die in COVID en Long COVID verantwoordelijk zijn voor een peptidestorm die de integriteit van het vaatweefsel aantasten (Between two storms, vasoactive peptides or bradykinin underlie severity of COVID?, Physiological Reports 2021 Mar 9(5)).

Endotheelcellen (ECs) genereren IL-1, IL-6 en CXC-chemokines. Dit stimuleert mestcellen om histamine vrij te maken, waardoor IL-8 en IL-6 worden bevorderd. Macrofagen in de longblaasjes worden door SARS-CoV-2 geactiveerd via de TLR. In reactie hierop maken macrofagen op hun beurt IL-1 aan, om mestcellen te stimuleren om nog meer IL-6 te produceren. Activering van de FceRI-receptor op mestcellen door IgE-antilichamen zorgt ervoor dat histamine wordt vrijgelaten. De afgifte van histamine en activering van de H2-receptor (Histamine-2) genereert cAMP en bevordert de productie van caspase-1, gevolgd door de productie van IL-1 in macrofagen en de synthese van IL-6. De ontsteking kan slechts gedeeltelijk worden geremd door inhibitie van granulocyten
(Mast cells activated by SARS-CoV-2 releases Histamine which increases IL-1 levels causing cytokine storm and inflammatory reaction in COVID, Journal of Biological Regulators and Homeostatic Agents 2020, Vol. 34, Issue 5).

Inhibitie van de H2-receptor en toediening van enkel een H1-blocker zijn ineffectief om MCAS te behandelen. Een voorgestelde behandelmethode is het blokkeren van de H4-receptor. De H4-receptor stimuleert mastcellen via de MAPK-route voor transport van eosinofielen (afweercellen); ook bevordert H4R de ontstekingsreactie via T-cellen en dendrieten. Een tweede voorstel is de toediening van stabilisatoren. Mestcelstabilisatoren zoals hydrocortison en clarithromycin kunnen de afgifte van histamine en cytokinen remmen. Mogelijke behandelmethoden voor specifieke mestcelinhibitie zijn de toediening van JAK1/JAK2-remmers en inhibitie van FcR1 door butyrate.

Interessant is een studie uit 2021 die aantoont dat Cromolyn en Cetirizine de ontstekingsreactie onderdrukken door CD45+-cellen in het hartweefsel te reduceren. Blokkering van de H1-receptor werkt in deze studie preventief tegen "lekkend" vaatweefsel. Daarentegen wordt de ontstekingsreactie nog wel opgewekt door chymase, tryptase en leukotrienen, de cytokinen die door mestcellen onafhankelijk van de H1-receptor worden vrijgelaten (Mast Cells are the Trigger of Small Vessel Disease and Diastolic Dysfunction in diabetic obese mice, Arteriosclerosis, Thrombosis and Vascular Biology 2021;41).

Lokale hypoxie kan resulteren in de afgifte van Weibel-Palade-lichamen (WPB) door het endothelium. Bestanddelen van WPB zijn de von Willebrand Factor (vWF) en P-selectin. Hierdoor worden bloedplaatjes en leukocyten in het endothelium geactiveerd. Bloedplaatjes bieden hun oppervlak aan aan stollingsfactoren, terwijl leukocyten Neutrophil Extracellular Traps (NETs) vormen. In dit milieu kunnen de anti-trombotische eigenschappen van mestcellen niet verhinderen dat de pro-trombotische eigenschappen van mestcellen bijdragen aan het ontstaan van Diepe Veneuze Trombose (DVT) (Mast Cell Granular Contents are Crucial for Deep Vein Thrombosis in Mice, Circulatory Research September 29, 2017).

2.6 NETs (Neutrophil Extracellular Traps)
Om virussen en andere pathogenen weg te werken, worden neutrofielen geactiveerd. NETs vormen een "val" van chromatinedeeltjes, met het doel om granulen van eiwitten los te laten op het virus. NETs worden gevormd door vezels van geactiveerde neutrofielen, verkorte DNA-histonen ("decondensed DNA" verwijst naar mitose-exit), chromatine uit de celkern, elastase, cathepsin-G en MPO.

Neutrofielen (NTs) reageren op ontstekingscytokinen, chemokines, interferonen (IFN) en DAMPs van virussen en andere pathogenen. In de vorming van NETs zijn vervolgens neutrofielelastase (NE) en PADA4 betrokken, die chromosomaal DNA en gasdermin D vrijlaten. Het is de bedoeling dat de celkernen van neutrofielen hiermee beschadigd raken om NETs te kunnen vormen. Een belangrijke functie van NETs is de inactivering van virussen door cathelicidins en MPO. NETs stimuleren daarnaast het immuunsysteem door IFN in dendritische cellen (DCs) te activeren en T-lymfocieten te activeren. In reactie hierop kunnen virussen de aanmaak van IL-10 stimuleren om de activering van NTs via de TLR-route tegen te houden.

In het proces van NET-vorming kan het weefsel echter schade oplopen. De vorming van NETs wordt geassocieerd met een toename van de ontstekingsfactoren IL-6, IL-8/CXCL8, CCL5 en Platelet Factor 4. De keerzijde is dat NETs de aggregatie van bloedplaatjes, de vrijlating van cytokinen en complementfactoren stimuleert, terwijl geactiveerde bloedplaatjes en neutrofielen. NETs stimuleren bovendien de transformatie van epitheelcellen (epithelium-mesenchymal transition, EMT), waarbij de epitheelcellen worden afgebroken. Bovendien zorgen histonen voor polarisatie (negatieve lading van de celmembranen) waardoor lysis van de celmembranen, weefselschade en ontstekingen optreden (NETs and COVID: A new frontiers for therapeutic modality, International Immunopharmacology 104 (2022)).

NETs stimuleren epitheelcellen in de bronchiën om de ontstekingsfactoren (cytokinen) IL-8, TNF-a, IL-1α en IL-1β af te geven. Met name de expressie van het type IL-1 en IL-36 wordt door NETs gestimuleerd om een ontsteking van de luchtwegen te bevorderen (Neutrophil Extracellular Traps activate IL-8 and IL-1 expression in human bronchial epithelia, American Journal of Lung Cellular and Molecular Physiology 2020 Jul 1). Er ontstaan feedbackloops van IL-1β met NETs. Daaraan wordt bijgedragen door activering van NLRP3 in longmacrofagen door NETs. Tijdens de acute fase van SARS/COVID stimuleren NETs de aanmaak van METs, gevormd door pro-ontstekingsmacrofagen (M1).

De bloedplaatjesgebonden-TLR4 kan bovendien worden geactiveerd om neutrofielen te stimuleren in de vorming van NETs. In reactie op dit proces activeren NETs P-selectin, om daarmee nog meer bloedplaatjes te aggregeren en zo trombose-activiteit te veroorzaken. Het complementsysteem gaat via de C3-cascade interactie aan met Tissue Factor op het celmembraan van neutrofielen, om bloedplaatjes te activeren en NET-vorming verder te stimuleren (NETs and COVID: A new frontiers for therapeutic modality, International Immunopharmacology 104 (2022)).

Een opvallende bevinding is dat anti-NET-IgG en anti-NET-IgM (antilichamen) werden gevonden bij patiënten die in het ziekenhuis zijn opgenomen voor COVID. Deze anti-NET-antilichamen worden via een Lupus-achtige perifere route van de B-immuuncellen gevormd bij mensen met een "hyperimmune" reactie in de vroegste fase van de infectie met SARS-CoV-2. Serum-DNase wordt beperkt door de anti-NET-antilichamen. Het gevolg is dat NETs niet normaal worden afgebroken, wat bijdraagt aan trombose (Autoantibodies stabilize neutrophil extracellular traps in COVID, JCI Insight 2021;6(15)).

De afbraakproducten van NETs die blijven circuleren (cir-mtDNA, neutrophil elastase, myeloperoxidase) vormen DAMPs (Damage Associated Patterns), die de ontstekingsreactie blijven activeren. Er ontstaat zo een feedbackloop van ontstekingsfactoren en NETs (Persistence of NETs and Anticardiolipin Auto-Antibodies in Post-Acute Phase COVID Patients, Journal of Medical Virology 2022;1-9).

2.6.1. NETs en ANCA
in auto-immuunziekten, vasculitis en endotheliumschade
In 2017 is een review gepubliceerd waarin werd gesteld dat aanhoudende NETosis (de vorming van Neutrophil Extracellular Traps) en een daaropvolgende verstoorde afbraak van NETs wordt geassocieerd met de ontwikkeling van auto-immuunziekten (Neutrophil Extracellular Traps (NETs) in Autoimmune Diseases: A comprehensive Review, Autoimmunity Reviews Vol. 16, Issue 11, November 2017). Neutrofielen, IL-8, Neutrophil Cytoplasmic Antibody (ANCA) activeren de NET-formatie. ANCA, een antilichaam betrokken in de immuunreactie, activeert neutrofielen en monocyten via PR3 en MPO. Het nadeel is dat bij overactivering van monocyten en neutrofielen, degranulering van het vaatweefsel en schade aan endotheelcellen optreden (Vasculitides, Nephrology Secrets 2019).

NETs bevatten een grote hoeveelheid aan auto-antigenen; onder meer Annexin A1, dsDNA, citrullines, cathelicidin, Apolipoproteïne A1, MPO, PR3, Properdin, TF, MMP-8, MMP-9, LAMP-2 en HMGB-1. Dit maakt NETs het middelpunt van autoimmuunziekten, omdat NETs een "loop" in stand houden met auto-immuunontstekingen. Bovendien zorgt stimulering van NETs met TNF-α ervoor, dat complementfactoren C3a, C5a en C5b-9 (MAC) worden gegenereerd. Complementfactoren zijn grotendeels verantwoordelijk voor het in stand houden van trombose-activiteit.

NETs activeren inflammasomen, die nauw zijn betrokken bij ontstekingsreacties. PMA-NETs activeren caspase-1, vervolgens NLRP3, waardoor IL-18 en IL-1β worden vrijgelaten. Deze interleukinen/cytokinen stimuleren weer de aanmaak van NETs.
Door B-cel geactiveerde immuuncomplexen activeren neutrofielen door aan de Fc-gammareceptor
(FcRIIIb) te binden en NETs te stimuleren. Ook in dit proces ontstaat een feedbackloop die wordt versterkt doordat NETs in staat zijn om CD4+ T-cellen te bewerken.

Door activering van de TLR4 (in reactie op een virus, ontsteking of een aandoening zoals Antifosfolipidesyndroom, APS) worden bloedplaatjes overactief. Bloedplaatjes stimuleren op hun beurt neutrofielen door HMGB1 vrij te laten (Neutrophil Extracellular Traps (NETs) Take the Central Stage in Driving Autoimmune Responses, Cells 2020;9, 915).

2.6.2. De wisselwerking van NETs met IL-8
Een onderzoek uit 2019 naar de invloed van IL-8 en NETosis op de ontwikkeling van atherosclerose is wat mij betreft zeer relevant voor zowel SARS/COVID in de acute fase, als Long COVID.

IL-8 stimuleert ontstekingen via de Src, ERK, p38 en MAPK-route

IL-8 (CXCL8) stimuleert de ontstekingsreactie door te binden aan de receptoren CXCR1 en CXCR2. Opvallend is dat inhibitie van Src, ERK en p38 de IL-8-gestimuleerde NETosis via CXCR2 remt. Dit houdt in dat de activering van de IL-8/CXCR2-NETs-as via de Src- en MAPK-routes verloopt.
IL-8 versterkt de adhesie van monocyten aan endotheelcellen, een proces dat schade aan het vaatweefsel versterkt. Dit proces wordt waargenomen in atherosclerose. De TLR9-agonist stimuleert de aanmaak van NETs en NF-kB- p65 fosforylering. Een behandelmogelijkheid is het toedienen van een TLR9-antagonist of NF-kB-inhibitor, of een Scr-, ERK- of p38-inhibitor (NETs induced by IL-8 aggravate atherosclerosis via activation of NF-kB signaling in macrophages, Cell Cycle 2019 Vol. 18, no. 21).

NETs bevorderen de aanmaak van IL-8 in macrofagen via TLR-9/NF-kB p65
Stimulatie met Lipopolysacchariden (LPS) en PMA van NETs bevordert de aanmaak van IL-8 mRNA, IL-6 en IL-1β in macrofagen via de route van TLR-9/NF-kB p65 fosforylering.

COVID-neutrofielen: IL-8-gedreven degranulering stimuleert trombose

De neutrofielen van COVID-patiënten reageren door stimulering met IL-8 met MPO-, lysozyme, CAMP-, cathepsin S-, en TREM1-depletie. Dit duidt op een proces van degranulering. In rijpende neutrofielen van COVID-patiënten wordt wel een (normaal) verhoogd gehalte van Cathepsin G en ELANE gemeten; dit bekent dat de expressie niet verandert, maar dat een versterkt proces van degranulering verantwoordelijk is voor een tekort aan granulocyten, die verantwoordelijk zijn voor het op gang brengen van de immuunreactie op (virus)infecties.

Fibrinogenen (FGA, FGG en FGB) en de coagulatie-factoren FV, FX en FIX zijn verhoogd in reactie op COVID-neutrofielen. Dit duidt op een vicieuze cirkel van IL-8 en trombotische factoren, veroorzaakt door COVID (Self-sustaining IL-8 loops drive a prothrombotic neutrophil type in severe COVID, JCI Insight 2021;6(18)).

Wat zijn er voor behandelmogelijkheden om NETosis te remmen?
Uitschakeling van CXCR2 houdt NETosis tegen en zorgt voor verbetering van de toestand bij atherosclerose. Dit kan door een anti-CXCR2-antilichaam toe te dienen. De stimulering van ontstekingscytokinen door NETs kan worden behandeld met een TLR-9 antagonist, of NF-kB-inhibitie. De TLR-9-antagonist IRS869 onderdrukt de aanmaak van onstekingscytokinen IL-6, IL-8 en IL-1β. Inhibitie van NF-kB met BAY11-7082 heeft een sterker effect dan de TLR-9-antagonist (Neutrophil Extracellular Traps induced by IL-8 aggravate atherosclerosis via activation of NF-kB signaling in macrophages, Cell Cycle Vol. 18, Issue 21, 2019).

2.6.3 De prominentie van IL-8 in acute SARS: een opmaat naar IL-depletie in Long COVID?
Het profiel van COVID in de kritieke fase toont depletie en uitputting van CD4 en CD8 T-cellen, dominantie van overactieve neutrofielen, monocyten en macrofagen (markers: CD64, CD16, HLA-DR, CD11b en CD69) en overactivering van cytokinen (IL-8, IP-10 en MCP-1). Twee cytokinen worden excessief geproduceerd bij de ontstekingsreactie door COVID: IL-8 en CXCL1/GROa; het niveau van IL-8 en GROa in het longvocht van COVID-patiënten in de acute fase van SARS, is 200 keer zo hoog als dat van de ontstekingsfactoren IL-6 en TNF-a. 

IL-8 wordt geassocieerd met longfalen; de PaO2/FiO2 is meetbaar gereduceerd, wat duidt op een falende longfunctie.

Er zijn twee IL-8-remmers. Dit zijn het antilichaam HuMaxIL-8 en de inhibitor Reparixin (The Role of Interleukin-8 in Lung Inflammation and Injury: Implications for Management of COVID and ARDS, Frontiers in Pharmacology Vol. 12, Januari 2022).

3. Is er meer kennis over Long COVID ten opzichte van 2003 ( = SARS-1)?
Er is niet veel veranderd op het gebied van de kennis over de langetermijngevolgen van SARS. Zoals bekend is sinds die eerste epidemie van 2003, hebben COVID-patiënten multi-orgaanschade (MOF) als gevolg van trombose en fibrose, veroorzaakt door COVID.

Long COVID is het gevolg van (micro)trombose en schade aan het endotheel weefsel in de vaten en organen. Ook in "milde" gevallen (dat zijn COVID-gevallen waarbij de infectie mild lijkt te verlopen) worden microtrombose en fibrose aangetroffen en blijft het endothelium overactief, wat weer trombose-activiteit stimuleert. Zoals sinds 2003 bekend is, zijn beroertes, schade aan het hartweefsel, neurologische problemen, een verhoogde kans op Alzheimer en schade aan het DNA algemene gevolgen van SARS/COVID.

4. Zijn medische predisposities (overgewicht, hoge bloeddruk en diabetes) van invloed op Long COVID?
Nee. Grote cohortstudies vinden geen verband tussen een hoog BMI, Diabetes Type 2 en een hoge bloeddruk en het risico op Long COVID ("Risk of Clinical Sequelae after the acute phase of SARS-CoV-2 infection: retrospective cohort study", BMJ 2021;373).

5. Wat zijn de verschillen tussen man en vrouw?

Mannen hebben een hoog risico op een ernstig verloop van de acute fase van COVID, doordat de basale niveaus van IL-6, CRP, complementfactoren, coagulatie- en andere trombotische factoren verhoogd zijn bij het mannelijk geslacht.

Bij vrouwen wordt in de Long COVID-fase een lagere diffusiecapaciteit in de longen waargenomen, een objectief meetbaar symptoom. Spierzwakte en uitputting in de Long COVID-fase correleren bij vrouwen met een verhoging van IL-6.

Het gen voor de TLR-7 (Toll-like receptor-7) bevindt zich op het X-chromosoom. TLR-7 activeert de antivirale IFN-I-respons. 

6. Zijn er markers om Long COVID (objectief) te beoordelen?
Omdat COVID een bloedstollingsziekte is en beschadiging van het endothelium en verandering van het vaatweefsel tot gevolg heeft, is het belangrijk om niet alleen naar ontstekingsmarkers te kijken, maar ook naar biomarkers voor verandering van het vaatweefsel. Ten tijde van dit schrijven was de studie naar markers voor verstoring van het vaatweefsel als onderliggende stoornis in de Long COVID-groep nog in de pers (zie: Elevated vascular transformation blood biomarkers in Long COVID indicate angiogenesis as a key pathophysiological mechanism, Molecular Medicine (2022) 28:122).

Recentelijk is een ranglijst opgesteld voor de meest relevante biomarkers die duiden op verstoring van het vaatweefsel door COVID. Dit zijn als volgt:
1. ANG-1 (Angiopoietin-1);
2. P-sel (= P-Selectin);
3. MMP-1;
4. VE-Cad;
5. Syn-1;
6. Endoglin;
7. PECAM-1;
8. VEGF-A;
9. ICAM-1;
10. VLA-4;
11. E-SEL;
12. Trombomoduline;
13. VEGF-R2;
14. VEGF-R3

De prominente marker in deze ranglijst, Angiopoietin-1 (Angpt-1), is betrokken bij de regulering van de bloeddruk. Angpt-1 beschermt het vaatweefsel tegen ontstekingen en "lekkages van de vaten" en zorgt voor behoud van het endothelium van de vaten en organen.

Angiopoietin-2 (Angpt-2) zorgt daarentegen voor beschadiging van het vaatweefsel door de barrièrefunctie van het weefsel te doorbreken. Zoals ik in mijn bericht van 25 december 2020 aangaf, is Angiopoietin-2 (Angpt-2) een indicator voor de ernst van COVID (zie: The quest for solutions to COVID's thrombosis pandemic: endothelial glycocalyx dysfunction and mitochondrial dysfunction are starting points, 25 december 2020, paragraaf 1.1).

Hoewel Angpt-1 een beschermende functie heeft, is de verhoging ervan een indicator voor Long COVID. Dit houdt in dat het vaatweefsel op lange termijn te maken heeft met schade door COVID, waarvoor voortdurend een beroep moet worden gedaan op het herstellende mechanisme van Angpt-1. Dat de marker Angpt-1 in Long COVID verhoogd blijft, betekent dat de schade aan het endothelium door COVID onvoldoende wordt opgelost. 

De tweede belangrijke marker in deze ranglijst is. P-Selectin. P-Sel faciliteert het transport en de aanhechting van bloedplaatjes, is betrokken bij de vroege ontstekingsreactie. P-Selectin faciliteert de vorming van NETs (Neutrophil Extracellular Traps), formaties van neutrofielen (zie: Pathways to deterioration in SARS-CoV-2: coagulation disorders driving thromboinflammation and pulmonary fibrosis in COVID, 6 mei 2020).

Opvallend is dat het niveau van P-Sel in Long COVID lijkt te duiden op een voortdurende verhoging van het niveau lymfocyten (witte bloedcellen) en niet op een aanhoudende aggregatie van bloedplaatjes.  

7. Is er een overeenkomst met ME?
In 2020 zijn Long COVID-patiënten zelf naar voren gekomen met de gedachte dat ME (Myalgische Encefalo-myelitis). De symptomen van Post-COVID-Syndroom/Long COVID en ME kunnen gelijkenis vertonen, maar de biologische markers zijn verschillend. Dit maakt dat de behandeling anders is.

In het onderzoek naar biologische markers voor Long COVID zijn 14 markers in een ranglijst geplaatst, zie de ranglijst onder paragraaf 6. Alle 14 markers zijn verhoogd in het geval van Long COVID. Bij ME is er daarentegen géén verhoging in de serumniveaus van P-Sel en onder meer de adhesiemoleculen (ICAM) en de VEGF (Vascular Endothelial Growth Factor).

The sickest patients with COVID-19 suffer a hyperinflammatory state-a cytokine storm-that has features in common with a rare haematological condition called haemophagocytic lymphohistiocytosis. A comprehensive clinical picture of COVID-19: inflammation, coagulation, endotheliopathy and thrombosis

The one question of the COVID-19 pandemic is:
Why are thrombosis and thromboembolism persistent, in spite of adequate thromboprophylaxis and anticoagulation?

The answer is: endothelial damage is a major factor that keeps on driving thrombotic events. Meanwhile, the practice of prophylactic and therapeutic doses of anticoagulation to treat COVID thromboembolism might have actually been inadequate (see "A remaining challenge: hypercoagulability characterizing COVID-19, despite anticoagulation practices", 5 Oktober 2020, "A stubborn complication: the quest for solutions to COVID's thrombosis pandemic", 30 November 2020 and "The quest for solutions to COVID's thrombosis pandemic: endothelial (glycocalyx) dysfunction and mitochondrial dysfunction are starting points", 25 December 2020).

I was baffled to learn that as of January 2021, thromboembolism is still overlooked in COVID treatment trials. Any of the mechanisms of endotheliopathy, inflammation, hypercoagulation and thrombosis should be addressed in order to treat COVID-19 correctly. That is, COVID should be regarded an endothelial and hematologic disease. The basics of COVID-19 are understood, as its underlying mechanisms of endothelial damage and thrombosis, as well as its characteristic of impaired thrombolysis and fibrinolysis are well described since the 20th century. More recent studies, published between 2000-2019 by Gralinski and Levi shed light on urokinase pathways and crosstalks between inflammation and coagulation. In other words: contrary to popular belief (popular claims are mostly false), much is known about the COVID-19 mechanisms and pathologies. Just two main complications are timing and the occurrence of simultaneous cascades: too many events, too little time.

1.      From entry of SARS-CoV-2 to COVID-19: how it starts
1.1    TMPRSS2, NRP1, ACE2 and macrophage infiltration;
1.2    PRRs, PAMP, DAMPs, NETs and inflammasome NLRP3;
1.2.1 Hyperactivation of macrophages, neutrophils and NKs incite inflammatory auto-activation;
1.3    Complement system induction of thrombosis and endothelial damage;
2       Dysfunctional endothelial cells (ECs), adhesion moleculed, platelets and OXPHOS;
2.1    Platelet interaction with dysfunctional endothelial cells promotes thrombosis;

1    From entry of SARS-CoV-2 to COVID-19: it starts


1.1 TMPRSS2, NRP1, ACE2 and macrophage infiltration
Looking at a comprehensive picture of COVID-19, which I have drawn above, COVID develops along and crossing the lines of inflammation, coagulation, endotheliopathy and thrombosis. It starts with SARS-CoV-2 binding the ACE2 receptor, infiltrating macrophages and activating interferon in epithelial cells in order to upregulate ACE2 to enhance cellular uptake in cells. ACE2 alone does not explain the coronavirus' success to penetrate cells. While other entry sites might be discovered within the next months, two compontents are certain co-factors of cellular uptake of the coronavirus: TMPRSS2 (the primer) and NRP1. Both components are used to enhance cellular uptake of SARS-CoV-2 through ACE2.

SARS-CoV-2 is not found to directly infect endothelial cells, but does cause endothelial cell dysfunction through direct binding of platelet ACE2, infection of macrophages, megakaryocyte hyperactivation of platelets, activation of PAMP and DAMPs and inflammatory NLRP3 involvement through Toll-like receptor-4 (TLR-4).

1.2  PRRs, PAMP, DAMPs, NETs and inflammasome NLRP3
Upon infection, pattern recognition receptors (PRR), such as Toll-like receptors (TLR) detect viral RNA and Lipopolysaccharide (LPS), pathogen-associated molecular patterns (PAMP). PRR signals interferon regulatory factors (IRF) and NF-kB. PAMPs mediate the breakdown of the endothelial glycocalyx. PAMPs induce Tissue Factor through monocytes. Subsequently, cytokines such as TNF and IL-1β induce Tissue Factor to generate thrombi, activate platelets and fibrotic factors. Activated neutrophils release histones and DNA in neutrophil extracellular traps (NETs). NETs can act as an amplifier for IL-1α-induced endothelial damage (COVID-19 is, in the end, an endothelial disease, European Heart Journal Vol. 41, Issue 32, 21 August 2020, P3038-3044).

In response to cellular damage, damage-associated molecular patterns such as proteins, oxidized phospholipids and DNA are released (DAMPs). DAMPs signal through TLRs, leading to coagulant and inflammatory amplification. DAMPs and PAMPs induce IL-1-autoinduction, a self-amplification loop that induces TNF and IL-6. Being recognized by TLR-4, PAMPs and DAMPs activate the NOD pyrin domain-containing 3 inflammasome (NLRP3). NLRP3 cleaves procaspase-1 into caspase-1, which signals IL-1β and IL-18 release, leading to pyroptosis (inflammatory cell death) (Endothelial activation and dysfunction in COVID-19: from basic mechanisms to potential therapeutic options, Signal Transduction and Targeted Therapy (2020)5:293).

RAS imbalance and ROS further activate the NLRP3 inflammasome. The Bruton Tyrosine Kinase pathway (BTK), which controls macrophages and TLR-mediated activation of the inflammatory NF-kB, maturates IL-1β through NLRP3-mediated endothelial activation (The interplay between inflammatory pathways and COVID-19: a critical review on pathogenesis and therapeutic options, Microbial Pathogenesis 150 (2021): 104673).

1.2.1  Hyperactivation of macrophages, neutrophils and NKs incite inflammatory auto-activation

Hyperactivation of macrophages, neutrophils and Natural Killer Cells (NKs) contribute to the release of DAMPs and PAMPs, inciting auto-activation. Upon detection of DAMPs, TLR9 in endothelial cells can be activated. TLR9 activation is associated with MAPK activation, signaling of NF-kB and activation of ICAM and VCAM adhesion molecules. In addition, PAMPs are involved in activation of the complement system.

1.3  Complement system induction of thrombosis and endothelial damage
As I have discussed in August 2020, the complement system mediates the generation of Reactive Oxygen Species (ROS), activation of NETs, activation of a pro-thombotic, hypercoagulatory profile and endothelial dysfunction through induction of MASP-TAFI, Ultra-Large von Willebrand Factors (ULVWF), PAI-1, IL-1β, IL-8, TM, P-selectin, adhesion molecules MCP-1, E-selectin, ICAM and VCAM. In addition, extensive debris of SARS-CoV-2 complement MAC formations were found in the microvasculature. Thrombosis was accompanied by complement C5b-9 and C4d (Complement associated microvascular injury and thrombosis in the pathogenesis of severe COVID-19: A report of five cases, Translational Research, June 2020, Vol. 220).

2  Dysfunctional endothelial cells (ECs), adhesion molecules, platelets and OXPHOS

2.1 Platelet interaction with dysfunctional endothelial cells induces thrombosis
The SARS-CoV-2 Spike protein is found to bind platelet ACE2 to promote thrombosis in COVID (SARS-CoV-2 binds platelet ACE2 to enhance thrombosis in COVID-19, Journal of Hematology & Oncology 2020; 13:120). Inflammation, hypoxia and endothelial cell activation activate platelets and induce platelet apoptosis. Platelet activation and apoptosis induce the release of thrombotic factors/coagulants. Activated platelets enhance P-selectin expression, neutrophil activation, CCL2/3/7 and Interleukins IL-1β, IL-7 and IL-8, Tissue Factor and Hepatocyte Growth Factor. IL-1β increases endothelial cell permeability. Reactive Oxygen Species (ROS) and mitochondrial stress enhance platelet hyperactivation and apoptosis. Platelets bind to neutrophils; formation of Neutrophil Extracellular Traps (NETs) contribute to thrombosis.

2.1.1 Shifting towards a prothrombotic profile (Thromboxane, ADP, P2Y, PAI-1, TF, VWF, low ADAMTS-13)
A progressive kind of thrombocytopenia occurs in COVID-19: platelets are consumed by microclots, then surviving platelets are hyperactivated by megakaryocytes as a means of (over)compensation. Viral damage and damage caused by mechanical ventilation induce platelet activation and apoptosis, further contributing to thrombosis. Platelets react to endothelial cell dysfunction through the release of prothrombotic Thromboxane, ADP through activation of P2Y, PAI-1 and VEGF. Dysregulation of endothelial cells followed by exposure to P-selectin, Von Willebrand Factor and fibrinogen in its turn activates platelets to express Tissue Factor (TF). VEGF initiates a prothrombotic Tissue Factor amplification loop. While the release of Nitric Oxide (NO) by endothelial cells acts as an antiplatelet agent, NO is impaired as a result of ROS (Innate immunity during SARS-CoV-2: evasion strategies and activation trigger hypoxia and vascular damage, Clinical & Experimental Immunology 2020, 202: 193-209; Thrombocytopathy and endotheliopathy: crucial contributors to COVID-19 thromboinflammation, Nature Reviews Cardiology, 2020).

ADAMTS-13 cleaves Von Willebrand Factor Multimers to modulate thrombotic activity. In COVID-19, VWF antigen (VWF:Ag) levels are markedly increased, while ADAMTS-13 levels are decreased. This VWF:Ag to ADAMTS13 ratio is found to correlate to disease severity, with an overall increase of VWF and decrease of ADAMTS13 in the majority of COVID cases admitted to hospital. An imbalance in VWF:Ag to ADAMTS-13 enhances the hypercoagulable state, platelet hyperactivation and microthrombus formation in COVID-19 (The ADAMTS13-von Willebrand factor axis in COVID-19 patients, Journal of Thrombosis and Haemostasis, 23 November 2020).

2.1.2 Hypoxia-inducible factor-1-alpha (HIF-1a): differentiating between inflammatory and anti-inflammatory

Adenosine Triphosphate (ATP) is the energy supply of cells. Oxidative Phosphorylation (OXPHOS), the Tricarboxylic Acid Cycle (TCA) and glycolysis synthesize ATP; the process is known as "aerobic cellular respiration". During SARS-CoV-2 infection, genes regulating mitochondrial OXPHOS and TCA are downregulated. TCA products citrate, aconitate and fumarate are reported to be depressed in COVID-19 (Metabolic reprogramming and epigenetic changes in vital organs in SARS-CoV-2-induced systemic toxicity, JCI Insight 2021;6(2)).

The function of Hypoxia-inducible factors (HIFs) is to adapt to hypoxic circumstances in order to maintain "normoxic" conditions. In sepsis, macrophage HIF-1α is required to switch from Oxidative Phosphorylation (OXPHOS) to glycolysis (Immunometabolism and Sepsis: A Role for HIF?, Frontiers in Molecular Biosciences Vol. 6, September 2019, Art. 85). HIFs can either induce or reduce an inflammatory state, depending on hypoxic conditions. Prolonged cell stress followed by hypoxia, HIF activation and a decrease in ATP will enhance cell necrosis and inflammation (Hypoxia, HIF-1α and COVID-19: from pathogenic factors to potential therapeutic targets, Acta Pharmacologica Sinica (2020)0:1-8).

The upregulation of VEGF by HIF-1α increases vascular permeability and NET formation. Inflammation, the release of cytokines and thrombotic factors following SARS-CoV-2 infection contribute to local hypoxia. Low tissue oxygen levels incite an adaptive response through Hypoxia Inducible Factors (HIFs) in order to enhance energy and promote angiogenesis, the generation of new vessels. HIF-1 alpha reduces oxidative stress-induced apoptosis through relocation to the intermembrane space of mitochondria (HIF-1α protects against oxidative stress by directly targeting mitochondria, Redox Biology 2019 Jul;25).

HIF-1 and HIF-2 regulate endothelial cell adaptation through endothelial migration, growth and differentiation (Primary endothelial cell-specific regulation of HIF-1 and HIF-2 and their target gene expression profiles during hypoxia, FASEB Journal 2019 Jul; 33(7): 7929-7941). The release of TNFα causes upregulation of HIF-1α mRNA. A transition from HIF-1 to HIF-2 marks the shift from acute to prolonged hypoxia. Accumulation of HIF-1 alpha is hypothesized to occur due to increased expression of HIF-1 and inhibition of proteasome degradation under a hypoxic state. HIF-1 alpha may stabilize in macrophages following activation of Toll-like receptor 4 (TLR4). PAMP, cell death and DAMP (see 1.2) release upregulate HIF-1 alpha.

Interestingly, a 2009 study proves that HIF-1α upregulates ACE and downregulates ACE2 in the absence of a true hypoxic state, with significant elevation of Ang II, a process that was antagonized by telmisartan (Role of HIF-1α in the regulation of ACE and ACE2 expression in hypoxic human pulmonary artery smooth muscle cells, Lung Cellular and Molecular Physiology Vol. 297, Issue 4, October 2009). In addition, HIF-1α is shown to upregulate ADAM17. This process is hypothesized to exert anti-inflammatory properties. However, due to its highly inflammatory properties under hypoxia and sepsis, HIF-1α contributes to deterioration in COVID-19.

Expression of HIF-1α in alveolar epithelial cells induces inflammation via the NF-kB pathway and mediates cellular inflammation through CD4+, CD8+, IL-2 and TNFα (COVID-19-driven endothelial damage: complement, HIF-1 and ABL2 are potential pathways of damage and targets for cure, Annals of Hematology 2020 Jun 24:1-7). HIF-1α stimulates glucose transporters and Lactate Deydrogenase (LDH) and controls the expression of VEGF (Vascular Endothelial Growth Factor), FOXOa and CXCR4 (a T-cell receptor). HIF-1 and HIF-2 metabolism is hypothesized to be dedicated to decreased production of adenosine triphosphate (ATP) through OXPHOS, reducing ROS in the process (Molecular Basis of "Hypoxic" Signaling, Quiescence, Self-Renewal and Differentiation in Stem Cells, Anaerobiosis and Stemness 2016, p. 115-141).

When oxidative phosphorylation falls down due to SARS-CoV-2 infection affecting the mitochondria of the host cells, HIF wil come to aid to generate ATP via glycolysis. This, however, is an inadequate way to generate ATP, which will turn out detrimental under prolonged inflammatory and hypoxic circumstances. Aerobic glycolysis even enhances the pathology of COVID. Glycolysis via HIF-1α favors SARS-CoV-2 replication and induces monocyte cytokine production. Through this mechanism, the HIF-1α axis promotes monocyte-driven inhibition of T cell responses and epithelial cell death, worsening the clinical picture (Elevated Glucose Levels Favor SARS-CoV-2 Infection and Monocyte Response through a HIF-1α/Glycolysis-Dependent Axis, Cell Metabolism Vol. 32, Issue 3, September 2020, p437-446). A possible therapeutic target to inhibit HIF-1α-glycolysis in COVID-19 is glycolysis inhibitor 2-deoxyglucose (Diabetes, obesity metabolism and SARS-CoV-2 infection: the end of the beginning, Cell Metabolism 33, March 2021). Furthermore, HIF-1α agonist show a tendency for attracting NETs and phagocyte extracellular traps after stimulation with LPS (Hypoxia, HIF-1α and COVID-19: from pathogenic factors to potential therapeutic targets, Acta Pharmacologica Sinica (2020)0:1-8).

Thus: the net effect of HIF-1α is complex. While HIFs are indispensable for providing energy to cells under hypoxic circumstances, HIF-1α contributes to inflammation and most likely induces an inadequate T cell response. SARS-CoV-2 downregulates mitochondrial oxidative phosphorylation, which is followed by HIF-induced glycolysis, an inadequate way to generate ATP, which even enhances viral replication in COVID.

2.1.3 Antiphospholipid antibody formation
Upon endothelial dysfunction and Reactive Oxygen Species (ROS) generation following infection with SARS-CoV-2, the beta 2 glycoprotein (β2 GP1) becomes oxidized. The function of non-oxidized β2 GP1 is to control Von Willebrand Factor (vWF) platelet binding to the subendothelium. Under the circumstances of endothelial dysfunction and ROS generation, oxidation of β2 GP1 leads to antiphospholipid antibody formation (aPL). Platelet aggregation occurs through vWF and GPIb. Alpha-granules on platelets release platelet factor to enhance oxidized β2 GP1 and aPL. In addition, granules release ADP, thromboxane and GPIIb, inducing NET formation and fibrotic activity (COVID-19 as a blood clotting disorder masquerading as a respiratory illness: A cerebrovascular perspective and therapeutic implications for stroke thrombectomy, Journal of Neuroimaging 2020;30:555-561).

Antiphospholipid antibodies (aPL) are immunoglobulins, resulting from the interaction of phospholipids with Annexin, prothrombin, cardiolipin. aPL trigger thrombotic activity. Among antiphospholipid antibodies are lupus anticoagulant, anti-cardiolipin antibodies (aCL) and anti-β 2-glycoprotein I antibodies (β2GPI). Notably, antiphospholipid antibodies exert thrombotic/coagulant properties through inhibition of protein C pathways, inhibition of antithrombin and fibrinolysis. Although a brief report observes that aPL were detected in 47% of severely ill COVID patients (Brief Report: Anti-phospholipid antibodies in critically ill patients with COVID-19, Arthritis & Rheumatology 2020 Dec;72:(12)), a recent study shows a low aPL prevalence with regards to COVID thrombosis mechanisms. Moreover, aPL positive patients were not admitted to the ICU (Anti-phospholipid antibodies and immune complexes in COVID-19 patients: a putative role in disease course for anti-annexin-V-antibodies, Clinical Rheumatology 2021 Jan 19:1-7).

Upon a critical reading of two case reports, a conclusion must be drawn that preexisting thrombotic factors such as Factor V Leiden have not been given sufficient attention. Development of aCL antibodies during a septic state cannot be ruled out and the development of aCL independent from thrombotic events in COVID has not been explored (Clinically significant anticardiolipin antibodies associated with COVID-19, Journal of Critical Care 2020 Oct; 59). The presence of high-titer IgG antibody in severe COVID cases has not been proved yet (The coagulopathy, endotheliopathy and vasculitis of COVID-19, Inflammatory Research 2020 Sep 12:1-9; see also "Coagulopathy of COVID-19 and Antiphospholipid antibodies, Journal of Thrombosis and Haemostasis 2020 May 28"). To date, no consecutive studies on the prevalence and developmental course of aPL with regards to COVID coagulopathy and thrombosis have been carried out (Reality Check on Antiphospholipid Antibodies in COVID-19-Associated Coagulopathy, Arthritis & Rheumatology Vol. 73, Issue 1, January 2021).

2.2 ADAM17 promotes ACE2 downregulation and subsequent induction of adhesion molecules through TNF-α
SARS-CoV-2 activates ADAM17, which activates TNF-α and subsequently downregulates ACE2 levels. The ability of ACE2 to increase cardiopulmonaryprotective Angiotensin 1-7 (Ang 1-7) is impaired, resulting in damaging levels of Angiotensin II and increasing levels of Bradykinin. Imbalance of the Renin-Angiotensin/Kinin-Kallikrein-System (RAAS/KKS) leads to permeability of the vasculature ("leaking vessels"), Reactive Oxygen Species (ROS), prostacyclin (PGI2) impairment and nitric oxide impairment. Loss of nitric oxide and prostacyclin results in vasoconstricion, platelet overactivation, upregulation of mitochondrial stress, leukocyte adhesion and impairment of the NRF2 antioxidant function. ACE2 reduces LPS-induced endothelial cell death. ACE2 also induces IL1-β and TNF-α through inhibition of the Janus Kinase (JNK) and NF-kB inflammatory pathways. Loss of ACE2 by activation of ADAM17 and TNF-α promotes adhesion molecule VCAM-1 and chemoattractant MCP-1/CCL2 and enables production of TNF-α, IL-6, adhesion JAM-A and metalloproteases MMP-2 and MMP-9.

ROS and vessel permeability activate calcium, adhesion molecules and the NF-kB pathway, a key contributor to the inflammatory cascade. Ang II and ROS activate inflammatory pathways. The central inflammatory cascade comprises the IL-6/Janus Kinase/STAT pathway, pro-inflammatory interleukins IL-7, IL-8, IL-12 and IL-17, Interferon-gamma (IFN-y), Toll-Like Receptors TLR-3/7/8, TNF-α, IL-1-β, CCL-2/3/7, the MAPK pathway and MIP1a. NOX2 further contributes to generation of ROS. ROS and vascular permeability activate calcium signaling and the NF-kB pathway to promote inflammatory cytokines and adhesion molecules. While TNF-α induces mitochondrial ROS via calcium influx, IFN-y upregulates mitochondrial ROS. Mitrochondrial ROS induces inflammatory cytokines to contribute to the inflammatory cascade.

3   Amplification of inflammatory and coagulation cascades promote thrombosis

3.1  IL-6 promotes adhesion molecules and enhances endothelial permeability through JAK/STAT

IL-6 is known to promote synthesis of coagulation factors fibrinogen, Tissue Factor and factor VIII. IL-6 prompts megakaryocytes to develop platelets and increases vascular permeability through endothelial cell activation of VEGF. IL-6 activates the Janus Kinase/STAT pathway, thereby induces upregulation of adhesion molecules VCAM, ICAM-1, E-selectin, and MCP-1. IL-6 also reduces Nitric Oxide (NO) and increases oxidative stress, enhancing endothelial permeability (Cellular and oxidative mechanisms associated with Interleukin-6 in the vasculature, International Journal of Molecular Science 2017 Dec; 18(12): 2563). 

3.2  Endothelial cell activation, amplification of inflammation and thrombosis

In the early stage of COVID-19, C reactive protein (CRP), an acute-phase reactant, increases. CRP is an inflammatory marker, created under control of IL-6 (High-sensitivity C-Reactive Protein, Chapter 18: Immune Function Assessment, in: Textbook of Natural Medicine (Fifth Edition), 2020 P157-165). CRP promotes endothelial cell apoptosis and induces EC activation. In addition, CRP acts on the NF-kB pathway to promote inflammation. When activated by IL-1β and TNF-α, endothelial cells induce pro-thrombotic Von Willebrand Factors (VWF), P-selectin and fibrinogen, thereby attaching platelets.  Platelets become activated by endothelial cells. Interacting with endothelial cells, platelets release VEGF, prompting endothelial cells to release Tissue Factor (TF).

Interferon-y activation through T-helper 1 cells (Th1), further stimulates cytokine production by endothelial cells (Endothelial activation and dysfunction in COVID-19: from basic mechanism to potential therapeutic approaches, Signal Transduction and Targeted Therapy 2020:5:293). Beside endothelial cell activation, damage of pericytes accounts for vascular permeability and amplification of the complement system, adding an amplification loop to the thrombotic mechanisms in COVID-19 (Innate immunity during SARS-CoV-2: evasion strategies and activation trigger hypoxia and vascular damage, Clinical & Experimental Immunology 2020, 202: 193-209; Thrombocytopathy and endotheliopathy: crucial contributors to COVID-19 thromboinflammation, Nature Reviews Cardiology, 2020).

Following SARS-CoV-2 infection, pneumocytes, alveolar cells, infiltrating monocyte-macrophages and neutrophils elicit TNF-α, IL-6, IFN-gamma and MCP-1. Macrophages start the amplification of inflammation, coagulation and thrombosis. Anticoagulant factors are downregulated by activated macrophages; the anticoagulant protein C system is impaired by inflammatory cytokines.More so, CD68+ macrophages are shown to be directly infected by SARS-CoV-2 (Immunity, endothelial injury and complement-induced coagulopathy in COVID-19, Nature Reviews Nephrology 17, 46-64(2021)). Adhesion neutrophils transforming to NETs amplify endothelial damage through IL-1α. Disruption of the endothelial barrier and ROS, IL-1β, TNF-α and NETs promotes Tissue Factor expression.

Tissue Factor activates the coagulation cascade through amplification of Factor VII and X, which enhances thrombin formation. Sepsis with dysfunctional endothelial barrier stimulates autoamplification of coagulation and inflammation through Factor XII and Tissue Factor, activating platelets which in their turn release CD40 and amplify VEGF. Inflammatory amplification increases TF formation, downregulation of thrombomodulin and leukocyte activation. IL-6 increases vascular permeability and promotes endothelial cells to amplify cytokine cascades.

4  Damage to the endothelial glycocalyx, impaired antioxidant activity, sepsis and impaired shear stress

The glycocalyx is a gel-like layer of sialic acid-glycoproteins, heparan sulphate, chondroitin sulphate, hyaluronans (bound to endothelial cells by CD44) and proteoglycans such as syndecans. Glycocalyx sialic acids express Nrf2 in order to exert antioxidant properties counteracting ROS (Glycocalyx sialic acids regulate Nrf2-mediated signaling by fluid shear stress in human endothelial cells, Redox Biology 2021 Jan;38: 101816). The junctions of the endothelial glycocalyx are stabilized by cadherin, claudins and platelet endothelial cell adhesion molecules (PECAM). The fluid layer of the glycocalyx is able to hold antithrombin, albumin and antioxidants. PAMPs damage the integrity of the glycocalyx-endothelial cell barrier, which is observed by shedding of syndecan-1, claudin-5, hyaluronans and syndecan-4 in COVID-19 patients.

Following infection, damage to the endothelial glycocalyx promotes sepsis and coagulopathy, which eventually leads to Disseminated Intravascular Coagulopathy (DIC). PAMPs activate the complement system, induce inflammatory amplification and promote hypercoagulation. Vascular permeability (through ROS, oxidative stress, release of Ang II, impairment of Nitric Oxide, NRF2 loss and PGI2 impairment) dysregulates normal shear stress. Blood flow and shear stress are necessary to maintain the balance between thrombosis/antithrombosis and oxidative/antioxidant activity in order to protect the glycocalyx and endothelial function. Low shear stress and impairment of blood flow shifts endothelial homeostasis towards a thrombotic and sepsis coagulopathy. MMP-9 produced by IL-1β induces the breakdown of the glycocalyx. As a result, an amplification loop of sepsis, glycocalyx shedding, endothelial dysfunction, leaking vasculature and low shear stress increase inflammation and thrombotic activity.

PAMPs activate coagulation through Tissue Factor (TF). Inflammatory cytokines such as IL-6, TNF-α, IL1-β further induce TF. Tissue Factor activates Factor VII, which in its turn activates Factor X with subsequent thrombin and fibrin generation. Activated endothelial cells release P-selectin and VWF to increase TF expression. Low shear stress and loss of shear stress induce red blood cell (erythrocytes) aggregation (The role of endothelial shear stress on haemodynamics, inflammation, coagulation and glycocalyx during sepsis, Journal of Cellular and Molecular Sciences Vol. 24, Issue 21, November 2020, P12258-12271).

5  Possible therapeutic targets

The following therapeutic targets for treatment of COVID-19 comprise inflammatory factor inhibitors,  thrombin inhibitors, interleukin inhibitors, Toll Like Receptor expression inhibitors, bradykinin inhibitors and Janus Kinase Pathway inhibitors, Nuclear Factor NF-kB inhibitors, as well as pharmacotherapeutic antioxidants and Vascular Endothelial Growth Factor (VEGF) inhibitors. Endothelial (glycocalyx) specific therapeutic options

Vascular leakage-increasing Angiopoietin-2 (Angpt-2) is significantly increased in COVID patients admitted to the ICU for mechanical ventilation, indicating endothelial damage. Tie2 activation is postulated to protect and restore endothelial glycocalyx (eGC) (Tie2 Activation Promotes Protection and Reconstitution of the Endothelial Glycocalyx in Human Sepsis, Journal of Thrombosis and Haemostasis 2019 Nov;119(11):1827-1838).

Another therapeutic option for endothelial glycocalyx restoration is Sulodexide (SDX) ("Therapeutic Restoration of Endothelial Glycocalyx in Sepsis, Journal of Pharmacology and Experimental Therapeutics 2017 Apr; 361(1): 115-121). Recently, recombinant Antithrombin-gamma (AT-γ) is proposed as a therapeutic option for endothelial glycocalyx restoration (Newly developed recombinant antithrombin protects the Endothelial Glycocalyx in an Endotoxin-Induced rat model of sepsis, International Journal of Molecular Sciences, 2021 Jan; 22(1): 176).


Therapeutic targets for treating COVID: a selection (version January 2021)

I created this selection of possible therapeutic agents for treating COVID, based on current research
