Pathways to deterioration in SARS-CoV-2 (Part III): Central Nervous System: neuroinvasive potential of coronaviruses and neurological manifestations (extensive medical review)

In my message from March 10, 2020, I reported on the neuroinvasive potential of SARS-CoV-1 and other coronaviruses, a topic that has been well studied since 2003 (Dutch version: see "Verspreiding en complicaties van coronavirussen", 7 maart 2020). The neuroinvasive potential of SARS-CoV-2 has been underestimated since the beginning of this pandemic. Reasonably, it has been questioned whether neurological manifestations are a result of neuroinvasion by the pathogen SARS-CoV-2, or a result of thromboinflammatory events induced by SARS-CoV-2.

Due to a lack of autopsy specimens, viral replication of SARS-CoV-2 in the CNS cannot be confirmed. That is no reason to not raise awareness on the CNS and PNS invasive potential of SARS-CoV-2. Aside from direct neuroinvasion of the CNS, neurological manifestations evoked by inflammation and coagulation disorders (thrombosis and embolism) should be monitored closely. In this review, I will elaborate on the neuroinvasive potential of coronaviruses, neurological manifestations and mechanisms behind CoV-CNS infection. What is the point in drawing a distinction between neurological manifestations that start with direct invasion of the CNS or CNS manifestations mediated by inflammation in the periphery (outside of the CNS)? It is of relevance in respect of therapeutic options and timing of medical intervention.

1. On the neuroinvasive potential of coronaviruses (2016-2020)
Like respiratory viruses in general, SARS-CoVs can cause damage to the central nervous system. Coronaviruses can induce encephalitis, as accumulation of interleukin-6 increases the permeability of the blood-brain barrier. The rapid replication of SARS-CoV-2 is a risk factor for initiating overreaction of the human immune system; viral meningitis can occur as a consequence (Human Coronaviruses and other respiratory viruses: Underestimated opportunistic pathogens of the Central Nervous System?, Viruses 2020, 12 (1), 14, published December 20, 2019). In a young patient group with CoV, a slight accumulation of IL-6, IL-8 and MCP-1 in the brain fluid was observed. MCP-1 is involved in initiating inflammatory responses in the brain (Coronavirus infections in the Central Nervous System and respiratory tract show distinct features in hospitalized children, Intervirology 2016, vol. 59, no. 3, published in February 2017).

In this 2016 study, a distinction was drawn between Coronavirus-CNS infection (CoV-CNS) and CoV-respiratory tract infection. The serum level of Granulocyte macrophage colony-stimulating factor (GM-CSF) was higher in CoV-CNS infected patients. GM-CSF has proinflammatory properties and plays a role in autoimmune encephalopathy. Levels of IL-6, IL-8, MCP-1 and GM-CSF were higher in cerebrospinal fluid samples of the CoV-CNS patient group. Lymphocyte and eosinophil counts were reduced in the CoV-CNS group, whereas CoV-respitatory tract patients had a significantly lower neutrophil cell count. The monocyte count in CoV-CNS was elevated. Notably, the serum level of Granulocyte colony-stimulating factor (G-CSF) was significantly elevated in CoV-CNS and CoV-respiratory infection alike. 

Autopsies performed during the SARS-CoV-1 epidemic revealed viral RNA in the cytoplasm of hypothalamic and cerebral cortical neurons, as well as brain edema and focal neuronal degeneration and neuronal necrosis, extensive glial cell hyperplasia and cellular infiltrates. No demyelination was reported. Interferon-gamma was expressed in glia, infiltration of CD68+ macrophages and CD3+ T-cells was observed in brain neurons and glia (The neurology of COVID-19 revisited: A proposal from the Environmental Neurology Specialty Group of the World Federation of Neurology to implement international neurological registries, Journal of Neurological Sciences 414 (2020) 116884). 

In ACE2 transgenic mice models, respiratory failure has been attributed to medullary infection with SARS-CoV. While little cellular infiltration was observed, the entry for viral infection of the brain was the olfactory nerve. One report on COVID-19 encephalitis reveals hemorrhagic rim enhancing lesions within the thalami, medial temporal lobes and subinsula. The potential for viral-induced vasculitis via brain endothelial ACE2-receptors increases the risk of cerebrovascular disease (COVID-19 and the nervous system, Journal of Neurovirology, 2020 May 23).



2. Neurological manifestations concerning viral infection
There are at least three categories of neurological manifestations associated with viral infection. Encephalopathy, inflammatory encephalitis, hemorrhagic and ischemic stroke are complications of systemic diseases, such as Multi Organ Failure, inflammation and coagulation. Direct viral invasion of the CNS results in encephalitis, meningitis and endotheliitis associated with cell necrosis and dysfunction. Acute Disseminated Encephalomyelitis (ADEM), Acute Necrotizing Encephalopathy (ANE), Guillain-Barré Syndrome and Kawasaki-like manifestations are immune-mediated post-infectious manifestations.    
 
2.1 Manifestations of the nervous system in SARS-CoV-2 patients
In a case study involving 214 patients with SARS-CoV-2, neurologic symptoms were seen in 36,4% of patients. Mean age of patients was 52 years. Of all reported patients with nervous system manifestations, 24,8% had CNS manifestations, 8,9% had Peripheral Nervous System manifestations and 10,7% had skeletal muscle injury. Typical Peripheral Nervous System symptoms are anosmia and ageusia (loss of smell and taste). Notably, 58,9% of patients with CNS manifestations had nonsevere infection. Neurologic manifestations occurred within 1-2 days of the infection. 2 patients were admitted to the hospital with SARS-CoV-2-induced hemiparese, of whom none had fever, cough or diarrhea. Of CNS manifestions, headache, impaired consciousness, acute cerebrovascular disease, coma, delirium, ischemic stroke and cerebral hemorrhage were reported.

2.1.1 Encephalopathy
In a study concerning 58 patients, 69% had agitation, 67% had diffusive corticospinal tract signs and 33% had dysexecutive syndrome consisting of attention difficulties, disorientation and poorly organized response movements. In 8 patients, enhancement in leptomeningeal spaces was noted; in 11 patients, bilateral frontotemporal hypoperfusion was noted. 1 of 8 patients had diffuse bifrontal slowing consistent with encephalopathy; in 2 patients, oligoclonal bands were present, protein and IgG levels were elevated in 1 patient. RT-PCR arrays of the Cerebrospinal Fluid (CSF) were negative for SARS-CoV-2. Pleocytosis was not observed in any of the cases. This study concludes that ARDS due to SARS-CoV-2 infection was associated with encephalopathy, agitation, confusion and corticospinal tract signs. However, no conclusions can be drawn with regards to inflammation-mediated encephalopathy, as no cytokine arrays were performed (Neurologic Features in Severe SARS-CoV-2 Infection, NEJM, 4 June 2020).

2.1.2 MRI findings in absence of specific CSF findings: unspecific encephalitis
A study investigating 50 patients reports that the most common finding was cortical signal abnormalities on Fluid-Attenuated Inversion Recovery (FLAIR). In 3 patients, accompanying subcortical and deep white matter signal abnormalities were present; abnormalities in the frontal lobe were present in 4 patients; parietal lobe in 3; occipital lobe in 4; temporal lobe in 1; insular cortex in 3 and cingulate gyrus in 3 patients. CSF array of 5 patients revealed total elevated protein in 4 patients. Cell count, albumin, glucose levels and IgG Index were within normal range, no oligoclonal bands were observed. Acute intracranial findings in the absence of cortical signal abnormality included 1 patient with acute transverse sinus thrombosis and 1 patient with acute infarction in the right middle cerebral artery territory. Of 2 cases without intracranial findings, elevated protein was found in the CSF. The main differential diagnosis of these findings is infectious or autoimmune encephalitis, seizure, hypoxia and hypoglycemia. The cases of bilateral frontal involvement may have hypoexia as underlying pathogenesis, as ARDS and frontotemporal perfusion are associated factors. Cortical microhemorrhage and breakdown of the Blood-Brain Barrier accompanied by hypoxia are thought to result in the observed imaging pattern. Overall, no conclusions can be drawn with regards to viral or immunity-mediated encephalitis (Brain MRI Findings in Patients in the ICU with COVID-19 Infection, RSNA Radiology, 8 May 2020). 

2.1.3 Unusual microbleeds
MR images of 9 patients with a common pattern of unusual findings are reported. All patients suffered from severe hypoxemia secondary to ARDS, with a delayed recovery of consciousness after sedation. None of the patients were reported to have Diffuse Intravascular Coagulation (DIC); all patients presented a hyperfibrinemic state as well as septic or inflammatory coagulation. Microbleeds were observed in unusual distribution with a specific tendency for the corpus callosum. Other locations of microbleeds were the posterior or anterior limbs of internal capsule and middle cerebellar peduncles. In the majority of the patients, subcortical regions were affected. These findings are assessed to be distinct from hypertension and amyloid angiopathy. The involvement of the corpus callosum is inconsistent with hypertension and amyloid angiopathy; short episodes of hypertension occurred in 2 of 9 patients, who were successfully treated with antihypertensives. It is hypothesized that direct damage to the endothelium and vessels could be explanatory, as direct interaction between SARS-CoV-2 and the ACE2-receptor could lead to damage of the Blood-Brain Barrier and bleeding within the brain tissue. Some of the lesions observed on MR imaging were reported to have a lineal shape resembling a vessel, which could represent microthrombi within vessels, consistent with hypercoagulation disorders and microthrombi in pulmonary vessels of other patients. Lastly, diffuse axonal injury or lipid embolism are explored as possible explanations (Unusual Microbleeds in Brain MRI of COVID-19 Patients, Journal of Neuroimaging, 8 July 2020).

2.1.4 Systemic hyperinflammatory CNS dysfunction
1 patient without comorbidities presented with left-sided paresthesias, a 1-day history of headache and ipsilateral paresis progressing within 12 hours to confusion and agitation. CSF showed lymphotic pleocytosis and increased proteins. PCR of SARS-CoV-2 in CSF returned negative, but positive in the nasopharyngeal swab. A second patient presented with agitation, difficulties naming objects, temporospatial disorientation and confusion. CSF showed lymphotic pleocytosis and increased proteins. In both patients, IL-6 was elevated, in one patient IL-1β was significantly elevated. In addition, CSF showed elevated levels of ACE in both patients. It is hypothesized that these cases of encephalitis are triggered by a cytokine storm. Treatments targeting IL-6 (tocilizumab) and IL-1 (anakinra) could be of benefit to treat CNS dysfunction (Increased CSF levels of IL-1β, IL-6 and ACE in SARS-CoV-2-associated encephalitis, Neuroimmunology & Neuroinflammation, 1 July 2020). 

2.1.5 Leukoencephalopathy and microhemorrhages
11 patients were indicated for brain MRI for persistently depressed mental status. Brainstem reflexes were preserved, response in extremities was diminished. None of the patients had overt Disseminated Intravascular Coagulation. CSF was available in only 1 patient and was negative for infectious or inflammatory meningitis and encephalitis and was negative for SARS-CoV-2 on RT-PCR array. Of 11 patients, 4 patients presented with diffuse leukoencephalopathy, 1 with microhemorrhages and 6 with both microhemorrhages and leukoencephalopathy. Patients with leukoencephalopathy had symmetric hyperintensities involving bilateral deep and subcortical white matter. Abnormalities were observed to extend from the precentral gyrus down to the centrum semiovale and corona radiata. At the temporal and occipital horns, the posterior cerebral white matter was involved in all patients, notably more than the anterior white matter. The deep grey nuclei were spared from leukoencephalopathy. Apart from the pre-central gyrus and occipital lobes, the juxtacortical white matter was spared from leukoencephalopathy. These findings are reported to be consistent with Delayed Posthypopoxic Leukoencephalopathy (DPHL), occurring 10-14 days after hypoxic insult and similar to a pattern in carbon monoxide poisoning victims. The sparing of juxtacortical white matter and deep grey nuclei rules out acute hypoxic ischemia. DPHL is likely related to oligodendroglial cell death and subsequent demyelination. Observed mild restricted diffusion in this study can be related to acute demyelination. Possible etiologies for the observed leukoencephalopathy are sepsis-associated, post-infectious demyelating or hemorrhagic encephalitis and posterior reversible encephalopathy syndrome.

Punctate microhemorrhages were seen in the juxtacortical white matter and corpus callosum, specifically the splenium. Similar microbleedings have been reported in cases concerning high-altitute exposure, related to hypoxemia and injury of the Blood-Brain Barrier. The observed microbleedings are reminiscent of axonal injury, but head trauma is ruled out. Two distinct pathogeneses are hypothesized: demyelination and disruption of the Blood-Brain Barrier. Both pathogeneses are related to hypoxia caused by SARS-CoV-2 (COVID-19-associated Diffuse Leukoencephalopathy and Microhemorrhages, RSNA Radiology, 21 May 2020).

2.1.6  Acute Necrotizing Encephalopathy (ANE)
A first report of presumptive COVID-19 associated acute necrotizing encephalopathy (ANE) presents one case in which testing for the presence of SARS-CoV-2 in CSF could not be performed. CT images reveal symmetric hypoattenuation within the bilateral medial thalami; brain MRI imaging reveals hemorrhagic rim enhancing lesions within the bilateral thalami, medial temporal lobes and subinsular regions. Basilar arteries and proximal posterior arteries are normal, while axial venogram demonstrates normal functioning of the cerebral venous vasculature and internal cerebral veins. ANE is a rare complication of influenza and other viral infections, being attributed to intracranial cytokine storms resulting in Blood-Brain Barrier damage, but in absence of direct viral invasion of the brain or parainfectious demyelination (COVID-19-associated Acute Hemorrhagic Necrotizing Encephalopathy: CT and MRI Features, RSNA Public Health Emergency Collection, Radiology 2020 Mar 31: 201187). 

A patient with a history of aplastic anemia with intermittent red blood cell and platelet transmission had not received immunosuppression recently. The patient presented with episodes of vacant staring, speech arrest, flexion of both shoulders and a brief tonic clonic seizure. A CT showed early swelling of the brain stem. New lymphopenia was observed. Thrombocytopenia consistent with aplastic anemia was treated with human leukocyte antigen-matched platelets. Repeat head CT showed increased hypodensity and swelling of the brain stem, new cortical and subcortical hypodensity in the occipital lobe and progression of brain stem swelling with pontine hemorrhage and symmetrical hypodensities in the deep grey matter and both amygdalae. CSF showed protein elevation, but normal white blood cell counts. CSF PCR for SARS-CoV-2 was negative; nasopharyngeal swab was positive for SARS-CoV-2.

MRI showed progression of extensive abnormal signal and hemorrhage in a symmetrical distribution within the dorsolateral putamina (situated in the basal ganglia), thalamic nuclei, subinsular regions, splenium of the corpus callosum, cingulate gyri and perirolandic regions. There was reduction of ventriculi, basal cisterns, temporal uncal herniation and moderate celebellar herniation. Severe thrombocytopenia contributed to the hemorrhagic components of encephalopathy. The patient was not lymphopenic before SARS-CoV-2 infection. Testing for interleukins was not possible. Direct invasion (neurotropic) of the CNS by SARS-CoV-2 is unlikely in this case, since the CSF PCR was negative. An immunostorm is a most plausible explanation for this medical picture of ANE (COVID-19-related acute necrotizing encephalopathy with brain stem involvement in a patient with aplastic anemia, Neurology Neuroimmunology & Neuroinflammation, 26 May 2020).


2.1.7 ANE successfully treated with steroids
A case of Acute Necrotizing Encephalopathy in a 51-year-old COVID-19 patient without a history of neurological diseases is possibly mediated by antibodies. He presented unresponsive, turning comatose, groaning and showing rhythmic movements of the right upper limb at day 21 of illness, while being weaned off oxygen. Subtle hyperintensities in bilateral thalami were observed. Blood and CSF revealed thrombopenia, lymphopenia and inflammation, CSF albumin-cytological dissociation with increased IgG antibodies and altered Blood-Brain Barrier. MRI showed progressing lesions with diffuse hyperintensity in the thalami, cerebellum, brainstem and supratentorial grey and white matter.

Regions of the fibre network, as ventral hippocampal commissure, brachium of the colliculus and stria medullaris appeared strongly stained. Bilateral distribution of abnormalities to the brainstem, thalami, cerebellum and white matter, hyperintensities and damage to bilateral thalami is distinct in ANE. This is different in clinical pictures of ADEM, which are defined by asymmetrical lesions with ill margins, mainly to be observed in periventricular regions and basal ganglia, with gadolinium enhancement. Early treatment with steroids and polyvalent immunoglobulin has been highly effective. The suspected cause is host response to SARS-CoV-2, associated with IgG targeting a neuronal antigen in fibre tracts. The antibodies are hypothesized to target an autoantigen through molecular mimicry with the virus, while cell destruction, the release of large amounts of autoantigens stimulate self-reactive cells and may lead to self-reactive antibodies. Inflammation contributes to IgG production and alteration of the Blood-Brain Barrier (COVID-19-associated Acute Necrotizing Encephalopathy successfully treated with steroids and polyvalent immunoglobulin with unusual IgG targeting the cerebral fibre network, Postscript Letter, Journal of Neurology, Neurosurgery & Psychiatry, 10 July 2020).

2.1.8 Meningitis/encephalitis associated with SARS-CoV-2 infection of the CNS
A February 2020 report describes a first case of a 24-year-old unconscious patient presenting with suspected meningitis/encephalitis associated with SARS-CoV-2 infection. Neck stiffness and seizures were observed. MRI demonstrated abnormal findings of medial temporal lobe, including hippocampus, suggesting encephalitis. In addition, significant paranasal sinusitis was observed, a finding warranting to pay attention to  retrograde synaptic transmission of SARS-CoV-2. While SARS-CoV-2 was not detected in the nasopharyngeal swab, CSF was positive for SARS-CoV-2. CSF lymphocyte count was slightly elevated (pleocytosis). Blood lymphocytes were decreased, white cell count was increased, neutrophils were dominant and C-reactive protein was increased. FLAIR imaging indicated right lateral ventriculitis and encephalitis on the right mesial lobe and hippocampus (A first case of meningitis/encephalitis associated with SARS-CoV-2, International Journal of Infectious Diseases, Vol. 94, May 2020).  

In the case of a 41-year-old patient with meningoencephalitis, no RT-PCR of the spinal fluid was taken, but administration of Hydroxychloroquine (HCQ) showed a remarkable improvement (Meningoencephalitis without respiratory failure in a young female with COVID-19 infection in Downtown LA, early April 2020, Brain, Behavior and Immunity, 2020 Jul; 87: 33).

2.1.9 ADEM: 2 cases of improvement after administration of Methylprednisolone or Dexamethasone
Acute disseminated encephalomyelitis (ADEM) is an autoimmune disease of the nervous system, a rare disease that often occurs after viral infections and that mainly affects children. However, cases of ADEM affecting adults following SARS-CoV-2 infection are reported. In one case involving a 64-year-old, the patient reported a loss of smell and taste, rapidly evolving into anosmia and ageusia, followed by bilateral vision impairment associated with sensory loss on the right leg. Relative afferent pupillary defect was detected and visual field tests showed defects bilaterally. Furthermore, irritability, headache, right abdominal sensory level and left-sided lower limb hyper-reflexia were detected. Brain and spine MRI revealed lesions of the brain, associated with one lesion at the eighth thoracic vertebra (T8) and bilateral optic nerve enhancement. CSF showed lymphotic pleocytosis, mainly presented by CD3+CD4+-T-cells and mild proteinorrachia (elevated proteins in CSF). Identical immunoglobulin G oligoclonal bands were present in CSF and serum. SARS-CoV-2 was detected in CSF. Serum revealed anti-SARS-CoV-2 IgG. The serum was negative for antiaquaporin-4 (AQ4) antibody and antimyelin oligodendrocyte glycoprotein (MOG), making a Neuromyelitis optica spectrum syndrome less likely. Treatment with methylprednisolone associated with IV immunoglobulins progressively improved the symptoms associated with ADEM (Acute disseminated encephalomyelitis after SARS-CoV-2 infection, Neuroimmunology & Neuroinflammation, 1 June 2020).

A 54-year-old presented with an episode of lost consciousness and prolonged anosmia and ageusia. RT-PCR for SARS-CoV-2 was positive. X-ray revealed interstitial pneumonia. Blood tests showed moderate lymphocytopenia and mild elevation of inflammatory indices (white blood cells, C-reactive protein, Fibrinogen). Upon sudden deterioration, a Blood Gas test was performed to reveal severe hypoxia. An EEG revealed two seizures started from the right frontotemporal region, diffusing in homologous contralateral hemisphere.

On MRI, multiple hyperintense lesions of the periventricular white matter were observed, without restriction or diffusion nor enhancement. Similar lesions were found in the bulbo-medullary junction, the cervical and spinal cord. CSF RT-PCR for SARS-CoV-2 returned negative.

In this case, the demyelination is thought to be sustained by a delayed immune response after the viremia. Systemic Inflammatory Response Syndromes (SIRS) or SIRS-like syndromes in SARS-CoV-2 patients can occur due to a cytokine storm, in which IL-1, IL-6 and Tumor Necrosis Factor-alpha (TNF-α) may be responsible for activation of glial cells, with subsequant demyelination. It is hypothesized that pneumonia with hypoxia leads to an increased anaerobic metabolism that triggers neurological damage. The virus could alternatively trigger the production of antibodies against glial cells, post-infection, which is associated with Guillain-Barré. Administration of Dexamethasone allowed a progressive recovery of the pulmonary impairment (SARS-CoV-2 can induce brain and spine demyelating lesions, Acta Neurochirurgica 162, 1491-1494 (2020), 4 May 2020).

Non-typical ADEM: axonal injury associated with microthrombosis in the brain

Distinct from a classic ADEM, a non-typical ADEM describes the case of a 71-year-old, whose autopsy shows mild brain swelling and hemorrhagic lesions disseminated throughout the white matter of the cerebral hemisphere. One day before SARS-CoV-2 diagnosis, the patient developed acute kidney injury, likely secondary to shock state and respiratory failure and contrast administration. Levels of C-reactive protein, ferritin and IL-6 were significantly elevated. The brain lesions consisted of foci of intraparenchymal blood disrupting the white matter, with macrophages at the periphery of the lesions. Generalized reactive gliosis was widespread in the white matter.

Damaged axons were highlighted at the periphery of the hemorrhages. A loss of myelin was identified, with macrophages and fragmented axonal processes within the lesions and oligodendrocyte apoptosis surrounding the lesions. Nearby hemorrhagic lesions, the tissue was relatively preserved. Subtle areas of subcortical white matter were seen with a variable perivenular distribution. Perivascular infiltrates were revealed to be macrophages and corresponding myelin loss was visible. The lesions showed a range of axonal injury, up to moderate axonal injury. Additional white matter lesions showed destructive lesions with central fibrin associated with forced out red blood cells, radiating loss of myelin around vessels. Axonal injury was marked at these microscopically identified lesions.

Microscopic cortical infarcts were identified with astrogliosis surrounding the infarcts. Necrotized neurons were scattered in the neocortex, CA1 hippocampal region and Purkinje cells in the cerebellum, indicative of hypoxic-ischemic injury. Scant perivascular T cells were seen, but no activation of B cells was observed. Immunostaining of the brainstem highlighted perivascular macrophages associated with arteriosclerosis. The white matter lesions are hypothesized to be vascular. Microscopic neocortical  infarctions are likely associated with microthromboembolism due to SARS-CoV-2 infection, with a role for the ACE2-receptor as a mechanism for the injury (Neuropathology of COVID-19: a spectrum of vascular and acute disseminated encephalomyelitis (ADEM)-like pathology, Acta Neuropathologica (2020): 140:1-6).

2.2 Markers for severe infection
Patients with severe infection had higher white blood cell counts, neutrophil counts, lower blood platelet counts, lower lymphocyte counts, increased C-reactive protein levels, high D-dimer levels, MOF, increased Lactate Dehydrogenase, Alanine Aminotransferase and Aspartate Aminotransferase levels, increased urea nitrogen, elevated creatinine levels and increased creatinine kinase levels. Patients with skeletal muscle injury had decreased lymphocyte counts and serious liver injury as well as kidney injury (Neurologic Manifestions of Hospitalized Patients with Coronavirus Disease 2019 in Wuhan, China, JAMA Neurology, April 10, 2020). Neurotropic infection of the Central Nervous System (direct virus invasion of the nervous system) is to be determined by CSF PCR. Postinfectious immune-mediated neurological disorders (such as ADEM and Guillain-Barré) can be distinguished from encephalitis by the absence of CSF pleocytosis, as well as elevated CSF protein.

3. Mechanisms

3.1 Penetration or avoidance of the Blood Brain Barrier and Blood Nerve Barrier
The ACE2-receptor (angiotensin-converting enzyme 2-receptor), is the host receptor for SARS-CoV-2. The binding of SARS-CoV-2 comprises S-glycoprotein cleavage by Furin and priming by transmembrane protease serine 2, TMPRSS2, located in the cell membrane. ACE2 has at least two distinct functions. In the RAS, the Renin-Angiotensin-Aldosterone System that regulates blood pressure, it cleaves Angiotensin I to Ang 1-9 and Angiotensin II to Ang 1-7, which functions as a vasodilator peptide. ACE2 also regulates the absorbance of aminon acids in the endothelium of the gut. ACE2 is found in the subfornical organ (SFO) in the brain, an area that regulates the cardiovascular system through fluid balance and hormone secretion. The SFO lacks the protection of the Blood Brain Barrier, which is why circulating peptides can reach the SFO. 

The Blood Brain Barrier (BBB) protects the Central Nervous System (brain and spinal cord) from direct interaction with the circulatory system. The BB consists of endothelial cells, a basement membrane, astrocytes and mural cells. Two main immune cells that interact with the CNS are perivascular macrophages and microglial cells. Blood-borne progenitors cross the BBB (The Blood-Brain Barrier, Cold Spring Harbor Perspectives in Biology, 2015 Jan; 7(1)). The targeting of immune cells constitutes immune-mediated inflammatory mechanisms that drive neurological manifestations. Viral replication and the release of cytokines increase the permeability of the BBB (Potential of SARS-CoV-2 to cause CNS infection: Biologic Fundamental and Clinical Experience, Frontiers in Neurology, 18 June 2020). 

SARS-CoVs are primarily involved in endotheliitis. This is associated with SARS-CoV's potential to directly infect the CNS by dysregulating the BBB though infection of the vascular endothelium. The main route is the ACE2 receptor expressed on endothelial cells. Avoiding the BBB or the Blood Nerve Barrier (BNB) in order to access the CNS is also possible when a virus enters dorsal root ganglia and autonomic ganglia (The neurology of COVID-19 revisited: A proposal from the Environmental Neurology Specialty Group of the World Federation of Neurology to implement international neurological registries, Journal of Neurological Sciences 414 (2020) 116884). 

3.2 Dissemination through neural olfactory connections
It is hypothesized that CNS invasion by SARS-CoV-2 affects respiratory function through damage of the medullary cardiorespiratory center. In transgenic rodent models, SARS-CoV was found to spread through the olfactory pathway to subcortical and cortical brain regions. The brainstem being infected by SARS-CoV sheds light on the possibility that the primary afferents of cranial nerves are involved in the invasion. Since no SARS-CoV has been detected outside the neurons of infected brain specimens, it seems likely that invasion of the CNS from peripheral nerves comprises synaptic connections between neural circuits. This should not be confused with central glial substance and cerebrospinal fluid (CSF) being possible routes for SARS-CoV-infection, since both reveal high ACE2 levels. SARS-CoV-2 could enter the olfactory pathway by infecting epithelial cells in the mucosa. Olfactory axons, the output from olfactory receptors, cross the cribriform plate in the ethmoid bone to end in the olfactory bulb and rhinencephalon. This mechanism, in which a virus infects neurons in the periphery in order to use axonal transport to invade the CNS, is called retrograde (axonal) transport. Noteworthy is the finding that high amounts of ACE2 and TMPRSS2 are expressed in the sustentacular cells of the olfactory epithelium.

Olfactory bulb-hypothalamus-hippocampus-thalamus-cortical-PAG-amygdala
The olfactory bulb projects to the olfactory nucleus, olfactory tubercle, amygdala, piriform cortex, peri-amygdaloid cortex, insula and the anterior end of the parahippocampal gyrus (Anatomy of the olfactory nerves, Nerves and nerve injuries, Chapter 18, Vol. 1: 2015, p. 273-276). The entorhinal cortex of the olfactory complex supplies afferent fibers along the entire length of the hippocampus. The olfactory system sends fibers directly to cortical regions, such as the frontal lobe, without synapsing in the thalamus (Cranial Nerve I: Olfactory Nerve, Textbook of Clinical Neurology, 2007, p. 99-112). The amygdala is part of a broad brain complex that includes the hypothalamus, hippocampus, orbitofrontal cortex and periaqueductal grey matter (PAG). The amygdala has reciprocal connections with the hypothalamus through the fornix and stria terminalis and with the PAG. It receives projections from the prefrontal cortex (Amygdala and hypothalamus: historical overview with focus on aggression, Neurosurgery Vol. 85, Issue 1, July 2019). 

The corticomedial (middle of the cortex situated) amygdala is connected to the anterior preoptic and ventromedial nucleus of the hypothalamus (note: the POA contains PGD2 (prostaglandin) synthase-expressing neurons). The central nucleus of the amygdala is connected to the lateral hypothalamus (see Grey's Anatomy 2nd edition, 2008 and 2010). The piriform cortex contains feedback circuits within the cortex and centrifugal axons to the ipsilateral (same side) olfactory bulb, as well as connections to the contralateral (opposite side) olfactory pathway (Olfaction and Taste, Architecture of the Olfactory Bulb, in: The Senses: a comprehensive reference vol. 4, 2008). Notably, ACE2 is found to be highly expressed in the piriform cortex, as well as in hypothalamic nuclei, ventricles, substantia nigra, amygdala, hippocampi and the frontal cortex (abundant information on ACE2 in the brain is found in the 2008 review "Angiotensin-converting enzyme 2 in the brain: properties and future directions", Journal of Neurochemistry, 2008 Dec; 107(6)). 

Nervus trigeminus and its neural pathway to the autonomous system
The largest cranial nerve is the trigeminal nerve (nervus trigeminus, V (= fifth) ), extended into mandibular, maxillary and opthalmic branches. It serves as a sensory and motor nerve. The trigeminal nuclei are located in the brain stem. Trigeminal neurons receive sensory information from facial mucosa. The afferent sensory fibers of the trigeminal nerve originate from cell bodies in the Gasserian ganglion. The sensory primary afferents synapse at the Trigeminal Brainstem Nuclear Complex (VBNC) that extends through the brainstem. The fibers terminate in the ventral posteromedial nucleus (VPM) of the thalamus.
How the Nervus Trigeminus (trigeminal nerve) is situated

The solitary tract, located in the medulla, is formed from the ganglion of the vagus nerve, glossopharyngeal and geniculate nerve. The solitary tract nucleus (NST) is described as the visceral nucleus of the brainstem. The nerve fibers departed from the NST synapse in the lateral and paraventricular hypothalamic nuclei, to project to the insula. The nucleus ambiguus (NA) is involved in motor functions of swallowing and speech. Both the NST and the VBNC are situated close to the cardiovascular center in the medulla oblongata, which regulates heart rate through nervous and endocrine control.

Conclusion
Like SARS-CoV-1 (2003), SARS-CoV-2 has neuroinvasive capabilities. Evidence of CNS invasion is available, but more material from autopsies and biopsies (tissue samples), imaging techniques (CT scans and MRI) will need to be analyzed to specifically assess damage done to the nervous system. In a number of cases there is evidence of direct invasion of the current coronavirus into the nervous system; this is confirmed by detection of SARS-CoV-2 in the cerebral spinal fluid by RT-PCR (CSF or cerebrospinal fluid). Direct CNS invasion by the coronavirus involves viral encephalitis, viral meningitis and viral endothelialitis (inflammation of endothelial tissue, resulting in, among other things, microbleeds in the brain). 
 
In other cases, due to systemic inflammatory mechanisms or a "cytokine storm" in response to SARS-CoV-2 infection, encephalopathy, encephalitis and Kawasaki-like manifestations are caused. Inflammatory encephalitis, an inflammation of the brain caused by the body's inflammatory response to SARS-CoV-2, is confirmed by testing the cerebral spinal fluid for pleocytosis and elevation of proteins. Nervous system manifestations that occur after infection are ADEM (Acute encephalmyelitis, with loss of white matter), ANE (Acute Necrotizing Encephalopathy, encephalitis with necrosis (rotting) of the brain tissue) and Guillain-Barré.

Why the distinction between direct neuroinvasion of SARS-CoV-2 or neurological manifestations caused by inflammatory mechanisms and immunological mechanisms in response to SARS-CoV-2? Direct neuroinvasion, e.g. viral meningitis, should be treated with antiviral therapy. In one case of SARS-CoV-2 meningoencephalitis, a patient was successfully treated with hydroxychloroquine (HCQ). If neurological manifestations are caused by immunological responses, for example Tocilizumab (IL-6 blocker) or Anakinra may provide improvement.

What to expect from COVID-associated CNS manifestations?

In the nearby future, more manifestations involving olfactory tract and retrograde transport will be observed, as well as SARS-CoV-2-associated encephalitis, non-typical ANE and post-COVID Guillain Barré.

The imaginary, brainless battle over the transmission routes of the coronavirus should be replaced by adequate measures to address every transmission route

Fomites vs. droplets vs. aerosols: a senseless battle
One cannot foster fools. Sadly, media provide full coverage to conspiracy theorists for the sake of short-lived news value regarding the issues of the day. Media who commit themselves to such kind of 'news distribution' are nothing more than a serving hatch. Unfortunately, influenceable people are not able to see through true (mostly monetary) motives of the pseudoscientists and cult leaders they choose to believe. It goes to show that a vast amount of people support their insincere intentions. Why is that? In turbulent times, it seems comforting to some to cling to the one who skillfully plays on the sentiment. Someone who knows exactly what people want to hear; a sounding board. And this sounding board knows how to engage his target group. He only needs a topic to exploit, in order to give his followers the impression that he knows what he's talking about. A topic that has been heavily exploited by pseudoscientists since May 2020, is the illusory debate on "airborne transmission" vs. "droplet and fomite transmission" of SARS-CoV-2. This debate does not contribute in any way to what really matters: prevention of transmission of SARS-CoV-2.

The imaginary battle among "transmission camps" was instigated by pseudoscientists and copy-pasters
The recent bizarre battle between "airborne transmission" versus "fomite (surface contamination) transmission" camps is solely an imaginary one. Transmission of SARS-CoV-2 takes place through multiple transmission routes. Excluding either transmission route is pointless. The debate about an adequate approach to coronavirus transmission has been hijacked and turned into a severely marginal discussion that lacks any scientific basis. 

The hijackers of the debate are not scientists nor visionaries, but pseudoscientists and obsessives who are also referred to as 'gurus'. What is really mean, is that these people, who falsely present themselves as 'scientists', have copied and pasted unverified, non-reviewed studies on airborne transmission of the virus and presented the results of their copy-paste-skills as their own hypotheses. Fabricated facts and circular reasoning are presented as "evidence" and used for political and personal monetary purposes. The frequently heard "I'm right" makes it obvious that it is not about science. It is neither interesting nor relevant to know "who was the first to discuss aerosol transmission of SARS-CoV-2" (really interested in the scientists who investigated aerosol transmission of SARS-CoV-1 and SARS-CoV-2?  Look up Van Doremalen, ir. L. Marr, Morawska and Bourouiba, search by name via NCBI (PubMed)).


The only topic of relevance: take adequate measures to stop the transmission of SARS-CoV-2
I have repeatedly brought this to the attention since March 2020: in order to limit transmission of the coronavirus, it should be acknowledged that measures must be taken against aerogenous transmission. Aerogenous transmission requires additional measures compared to transmission via fomites or direct droplets. Direct droplet infection occurs when large droplets travel over a distance of about 3 meters from a person. Plexiglass fences in public spaces and plastic face masks in medical care serve to ward off direct droplerts. Transmission via fomites takes place by contracting solid particles, for example poo and slime, on surfaces.

Aerosols are droplets that are emitted by breathing / talking / singing. The dividing line between direct droples and fomites cannot always be drawn, because poo and slime can also become airborne, for example through an air conditioner or vacuum cleaner that swings small particles into the air ('sling poo'). Aerosols are moved via gas and they are capable of being transported by air over a distance of 8 meters. Note that aerosols accumulate in closed spaces. No distance rule helps against the accumulation of viral aerosols in an enclosed space. Therefore, aerogenous transmission should be limited with face masks and good ventilation. The advantage of good ventilation and mouth masks is that these measures can also help to limit direct droplet transmission.

What is the importance of limiting aerogenous transmission of SARS-CoV-2?
1. SARS-CoV-2 can remain stable in the air for up to 3 hours (Van Doremalen, 2020; see also Morawska, 2020). Accumulation and duration of the presence of viral aerosols increase the chance of successful infection of individuals;
2. Of all viruses tested, SARS-CoV-2 is the most stable in aerogenous form. SARS-CoV-2 aerosols can be infectious for up to 16 hours (Persistence of SARS-CoV-2 in Aerosol Suspensions, Emerging Infectious Diseases, Vol. 26, Number 9, September 2020);
3. The airborne transmission route influences the severity of the disease caused by SARS-CoV-2. Aerosols are small enough to directly reach the alveoli (alveoli). This can make the infection more serious, because the virus is not detected and the body fails to trigger an immune response in time (Particle size and pathogenicity in the respiratory tract, Virulence, 4 (8) (2013), see also "COVID -19 vulnerability: the impact of genetic susceptibility and airborne transmission ", Human Genomics, 2020; 14:17).


The importance of taking adequate measures against airborne transmission of SARS-CoV-2
The importance of clear policies and good communication
Unfortunately, the policies issued by the Institute for Public Health, which are based on the considerations of the Outbreak Management Team (OMT), do not excel in clarity. The aerosol transmission of SARS-CoV-2 is still dismissed (as of July 2020) and the reasoning behind this biased dismissal is incomprehensible. A completely normal physical phenomenon is denied, while aerogenous transmission of pathogens has never been a controversial topic among engineers and biochemists. Moreover, the circular reasoning is in fact highly unscientific:

1. The non-validated preprint that discusses transmission routes of SARS-CoV-2 on the Diamond Princess indicates that aerogenous transmission was not the primary transmission route. The substantiation of the scientific relevance of the study expresses that good ventilation techniques should be recognized. The outdoor fresh air supply on the Diamond Princess operated on 50% to 100% outdoor air in shared spaces. The hypothesis that aerogenous transmission was not the main transmission route, can be explained by the application of adequate ventilation techniques. It is all the more remarkable that the Health Institute discards adjustment of ventilation policies while quoting specifically this paragraph of the Diamond Princess preprint;
2. According to the OMT and Public Health Institute, it is more plausible that fomite transmission is the main transmission route for all infections. Transmission of SARS-CoV-2 via fomites has in fact never been demonstrated. There is literally no scientific basis to support the assumption that contamination via fomites is the main route of SARS-CoV-2-transmission;
3. According to the Public Health Institute, the viral load of aerosols is "probably" not infectious, even though this assumption has not been proven;
4. Face masks are not recommended as they should not replace the 1.5 meter spacing rule. Why do the OMT and National Institute for Public Health present distancing and face masking policies as alternative opposites, instead of cumulative measures? 

The Diamond Princess preprint from MedRxiv: ventilation operated on 50 to 100% outdoor air in shared spaces. Nonetheless, the Public Health Institute quotes this exact paragraph to discard adequate ventilation techniques.


1.5 m social distancing and the "disappearing causality trick"
Subsequently, causality questions have been given the cold shoulder to "prove" that the 1.5 meter distance rule was the right measure to prevent further transmission of SARS-CoV-2: "Because we stick to the 1.5 meter distancing rule, there are so few infections" (search the archive of NOS.nl, Monday 22 June 2020, 11:14). That a "1.5 meter distancing"-rule is not an adequate measure against transmission should not be questioned. The OMT's arguments also disregard that measures should be maintained cumulatively.

The precarious policy considerations have given some citizens the impression that "freedom rights" are being deliberately violated. I have also spoken to benevolent people who, because of the many contradictions, are not sure what precautions and measures have to be taken. It is therefore important to clarify how to ensure adequate measures.

Which measures must be taken?
1. The 1.5 meter distancing rule is not sufficient. A safe distance to minimize direct droplet transmission of SARS-CoV-2 stars at 3 m between each person;
2. Non-medical face masks actually help to restrain direct droplet transmission and aerosol transmission. FFP2 / FFP3 / KN95 certified face masks protect both the wearer and others against transmission;
3. Plexiglass screens can ward off direct droplet transmission, but they should not be arranged so that aerosols can accumulate. Ventilation to refresh the air is required;
4. Arrows and duct tape are of no use as long as people are not required to wear face masks;
5. Queueing customers and patients without requiring them to wear face masks is of no use, as long as respiratory droplets can be inhaled in the process;
6. To disinfect surfaces, such as shopping trolleys, use an emulsion with at least 60% alcohol / ethanol. This emulsion should not be wiped off;
7. Hand hygiene is important to remove faecal virus particles. Hand soap or another degreasing soap helps destabilize SARS-CoV-2;
8. Ventilation is the most effective method of limiting the spread of SARS-CoV-2 via multiple transmission routes. Side notes are that people and objects should not be placed directly in an air stream and recirculation of polluted air through mechanical ventilation systems and leaks in HVAC systems should be avoided.

SARS-CoV-2: coagulatiestoornissen: de mechanismen achter tromboinflammatie (trombose-activiteit) en pulmonaire fibrose

Hemostatische disbalans in ernstige SARS-CoV-2-gevallen: trombotische activiteit
In deze serie bespreek ik de hemostatische disnbalans die kenmerkend is voor ernstige gevallen van SARS-CoV-2. Zoals kan worden geleerd van de SARS-CoV-1-epidemie (2003) en de enorme hoeveelheid kennis die sinds 2003 wordt uitgebreid, zou de focus bij verslechtering niet voornamelijk moeten liggen op "SARS als longziekte"; in geen geval mag de sleutelrol van stollingsstoornissen als reactie op ontstekingen worden onderschat. 


Het drievoudige mechanisme dat betrokken is bij zeer besmettelijke ziekten zoals SARS, bestaat uit hemostatische, inflammatoire en trombotische reacties, die recentelijk zijn erkend (The era of thromboinflammation: Platelets are dynamic sensors and effector cells during infectious diseases, Frontiers in Immunology, 13 September 2019). In dit medisch-technische bericht bespreek ik achtereenvolgens:

1. Voorspellende factoren voor SARS-CoV-2-geassocieerde trombo-inflammatie;
2. Mechanismen die ten grondslag liggen aan trombocytopenie;
2.1 Von Willebrand Factor- ADAMTS-13 (metalloprotease) mechanisme in trombotische trombocytopenische purpura (TTP);
3. Interactie van endotheliumschade en consumptie van bloedplaatjes;
3.1 NET's: Neutrophil Extracellulair Traps: overdrijving van een normaal ontstekingsproces?

1. Prognostische factoren van SARS-CoV-2-geassocieerde trombo-inflammatie (ontsteking)
1.1 Prognostische factoren voor de ernst van SARS-1- en SARS-2-gevallen

In een cohortonderzoek onder 191 ernstig zieke SARS-CoV-2-patiënten, laag aantal lymfocyten, ernstige lymfopenie, leukocytose (verhoogde witte bloedcellen), verhoogde alanineaminotransferase (ALT, ernstig verhoogd door een beschadigde lever), lactaatdehydrogenase (LDH), hoge gevoeligheid troponine I, creatininekinase, verhoogde D-dimeerwaarden, serumferritine, IL-6, verlengde protrombinetijd en procalcitonine werden waargenomen in gevallen van ernstige verslechtering (
Clinical course and risk factors for adult inpatients with COVID-19 in Wuhan: China: a retrospective cohort study, The Lancet, 9 March 2020). Verhoogde D-dimeerwaarden van meer dan het dubbele van de bovengrens van normaal is een prognostische marker voor het risico op veneuze trombo-embolie (Pulmonary embolism in patients with COVID-19: Time to change the paradigm of CT, Thrombosis Research, June 2020). Het niveau van inflammatoir IL-6 was extreem hoog bij ernstig zieke COVID-19-patiënten (Detectable Serum SARS-CoV-2 viral load (RNAaemia) is closely correlated with drastically elevated interleukin 6 (IL-6) level in critically ill COVID-19 patients, Clinical Infectious Diseases, 17 April 2020).

Overeenkomsten worden gevonden in een SARS-CoV-studie uit 2004. Voorspellende factoren voor respiratoir falen waren aanvankelijk absoluut aantal neutrofielen (ANC), piek CK-niveau, piek CRP-niveau. piek LDH-niveau en laagste aantal lymfocyten. De meeste patiënten hadden verhoogde C-reactieve proteïnespiegels en lymfopenie, andere veel voorkomende abnormale bevindingen waren leukopenie, trombocytopenie en verhoogde aminotransferase, lactaatdehydrogenase en creatininekinase
(Clinical Manifestations, Laboratory Findings and Treatment Outcomes of SARS patients, Emerging Infectious Diseases, May 2004).

In een onderzoek met 85 ernstige gevallen van SARS-CoV-2 had 81,2% van de patiënten bij opname significant lage eosinofielen (witte bloedcellen), 60% had neutrofielen boven het normale bereik, 77,6% van de patiënten had lymfocyten onder het normale bereik en 78,8% van de patiënten had albumine onder het normale bereik. Verhoogde procalcitonine van meer dan 0,5 was geassocieerd met een overlijdenskans van 93%. Er wordt verondersteld dat eosinofilopenie mogelijk verband houdt met uitputting van CD8 T-cellen, waardoor SARS-CoV-2-geïnfecteerde patiënten met lagere niveaus van IL-5 , een interleukine dat betrokken is bij de proliferatie van eosinofielen

1.2 Meest typische factoren geassocieerd met SARS-CoV-2-gerelateerd trombotisch risico
Bij opname vertonen patiënten met een ernstige progressie van SARS-CoV-2 verhoogde D-dimereniveaus en fibrine / fibronogeen-afbraakproducten, maar afwijkingen in de protrombinetijd, partiële tromboplastinetijd en het aantal bloedplaatjes komen niet vaak voor. Hoewel het aantal bloedplaatjes geleidelijk afneemt, is er geen bloeding gemeld, ongeacht het optreden van DIC. De hypothese is dat dit duidt op een lokale expressie van DIC, pulmonale vasculaire trombose met daaropvolgende activering van fibrinolyse. Longtrombose kan protrombotische endotheeldisfunctie veroorzaken, die een ontstekingscascade veroorzaakt via complement en cytokine-afgifte en bloedstolling met vasculaire microthrombose die lokale consumptie-coagulopathie veroorzaakt

Verhogingen in PT zijn beperkt, terwijl aPTT normaal is bij opname. 10 dagen na opname zijn progressieve DIC, verlaagd fibrinogeen, verhoogd D-dimeer en verhoogde PT gemeld. Het niveau van ontsteking bij opname wordt aangegeven door verhoogde niveaus van IL-6, gecorreleerd met verhoogde fibrinogenen
(COVID-19 and its implications for thrombosis and anticoagulation, American Society for Hematology, 27 April 2020). Trombocytopenie wordt in 12% van de gevallen gemeld. In 2003 was trombocytopenie de meest opvallende bevinding in ernstige klinische cursussen. Het aantal bloedplaatjes was progressief afgenomen in 90% van de ernstigste gevallen, wat erop wijst dat er gedissemineerde intravasculaire coagulatie optreedt na beschadiging van de pulmonale capillaire vliezen veroorzaakt door inflammatoire bloedplaatjesaggregatie en microthrombusvorming (Prognostic factors for SARS: a clinical analysis of 165 cases, Clinical Infectious Diseases, Vol. 38 Issue 4, 15 Februari 2004).

Trombocytopenie (laag aantal bloedplaatjes die bijdragen tot stolling na bloeding) en verhoogde D-dimeer (fibrine-afbraak) niveaus kunnen worden verklaard door overmatige activering van de stollingscascade en bloedplaatjes. Naast endotheeldysfunctie, activeren Von Willebrand Factor (VWF) activering, het vrijkomen van weefselfactoren en activering van de Toll-like receptor (TLR) homeostatische onbalans. 2. Mechanismen die ten grondslag liggen aan trombocytopenie bij SARS-CoV-2
Drie mechanismen waarmee coronavirussen het hematopoëtische systeem verstoren, worden verondersteld. Ze kunnen op elkaar inwerken. De eerste hypothese is dat SARS-CoV-2, net als andere coronavirussen, beenmergcellen en bloedplaatjes binnendringt via aminopeptidase CD13, aanwezig op epitheelcellen, en vervolgens groeiremming en apoptose induceert, wat leidt tot remming van hematopoëse (vorming van bloedcellen en bloedplaatjes), resulterend in trombocytopenie. Activering van het macrofaagsysteem (de rekrutering en afgifte van inflammatoire cytokines) verbruikt rode bloedcellen. Na de activering van T-cellen (transportcellen) veroorzaakt een ontstekingssoep met IL-6 immuunschade aan het longweefsel. Schade aan capillair weefsel verscheurt megakaryocyten (waarin bloedplaatjes worden geproduceerd) en blokkeert bloedplaatjes, waardoor de afgifte van bloedplaatjes in het longsysteem wordt belemmerd. Opgemerkt moet worden dat IL-6, SARS-eiwitten ORF3a en ORF8a (de eiwitten van SARS-CoVs) en een verscheidenheid aan cytokines bijdragen aan epitheliale en vasculaire permeabiliteit, waardoor de inflammatoire cascade verder toeneemt.

Een tweede verondersteld mechanisme is dat antilichamen op plaatjesoppervlakken worden gedetecteerd door het reticulo-endotheliale systeem (RES), een deel van het immuunsysteem dat zich in het endotheelweefsel bevindt. De vernietiging van bloedplaatjes is het gevolg van het feit dat bloedplaatjes worden bekleed met antilichamen tegen bloedplaatjes.
Een derde hypothese verklaart de algemene klinische bevindingen in ernstige gevallen van SARS-CoV-2. Beschadigde pulmonale endotheelcellen activeren bloedplaatjes in de longen, aggregeren microthrombi, gevolgd door consumptie van bloedplaatjes. Dit lijkt verenigbaar te zijn met DIC gezien in SARS-CoV-2-gevallen

Het is opmerkelijk dat wanneer de Von Willebrand-factor uit muizen wordt geslagen, er geen adenovirus-geïnduceerde trombocytopenie optreedt. Virusgeïnduceerde trombocytopenie hangt hoogstwaarschijnlijk af van de interactie tussen bloedplaatjes en Von Willebrand Factor, een stollingsfactor die betrokken is bij de hechting van bloedplaatjes aan het beschadigde subendohelium

2.1 Von Willebrand Factor- ADAMTS-13 (metalloprotease) mechanisme in trombotische trombocytopenische purpura (TTP)

De hemostatische functie van de Von Willebrand Factor (VWF), aangebracht op het subendotheel, is het rekruteren van bloedplaatjes voor beschadigde bloedvaten door te binden aan het bloedplaatjes GP Ib-IX-V-complex. VWF wordt opgeslagen in megakaryocyten / bloedplaatjes en in door histamine geactiveerde endotheelcellen. Na stimulatie van het endotheel worden ultragrote multimeren van de VWF (ULVWF) vrijgegeven, die stevig aan bloedplaatjes binden. Als dit mechanisme niet werkt vanwege een tekort aan ADAMTS-13, treedt trombotische trombocytopenische purpura (TTP) op. De verworven vorm van TTP is het resultaat van antilichamen die zijn gericht tegen ADAMTS-13. Een inherente 'zwakte' van ADAMTS-13 is de afwezigheid van een transmembraan domein; een oplosbare vorm van ADAMTS-13 hecht aan het A3-domein van VWF

ADAMTS-13 voorkomt de vorming van trombi. Deze metalloprotease is de sleutel bij het downreguleren van trombose en ontsteking. Een tekort aan ADAMTS-13 vormt op zichzelf geen TTP of ischemische beroerte, maar veroorzaakt wel dat de protromobotische toestand wordt versterkt door andere ADAM-metalloproteasen, cytokines en MMP's.


3. Interactie van endotheelbeschadiging en consumptie van bloedplaatjes 
Schade aan het endotheel weefsel wordt geassocieerd met Multisystem Organ Failure (MOF), zoals onlangs gemeld in een ernstig geval van COVID-19. Endotheliumdysfunctie wordt genoemd als de belangrijkste determinant van microvasculaire disfunctie, door te verschuiven naar verbeterde vaatvernauwing met daaropvolgende orgaanischemie, ontsteking met weefseloedeem en een pro-coagulante toestand. Bovendien wordt verondersteld dat inductie van apoptose en pyroptose een sleutelrol speelt bij endotheelcelletsel, waardoor de microcirculatie in vaatbedden wordt verstoord (Endothelial cell infection and endotheliitis in COVID-19, The Lancet, 20 April 2020). Beschadigd longweefsel en pulmonale endotheelcellen resulteren in de bloedplaatjesaggregatie in de longen, terwijl de vorming van trombi op de beschadigde plaats de consumptie van bloedplaatjes kan veroorzaken. Langdurige ventilatie kan longfibrose veroorzaken. Een verder verhoogde consumptie van bloedplaatjes en verminderde productie van bloedplaatjes kan leiden tot trombocytopenie (Thrombocytopenia in patients with SARS, Immune Hematology, April 2005; 10(2)).

3.1 NET's: extracellulaire 'vallen' van neutrofielen: overdrijving van een normaal ontstekingsproces?
Bij detectie van pathogenen bevorderen geactiveerde bloedplaatjes de extracellulaire vallen (NET's) van neutrofielen. NET's bevatten enzymen van chromatine, histon en granulaat die worden uitgestoten door geactiveerde neutrofielen. Dit proces heet NETosis. P-selectine, afgeleid van bloedplaatjes, vergemakkelijkt interacties van plaatjes-neutrofielen tijdens de vroege fase van het NETosis-proces. Het vrijkomen van cathepsine G en serineprotease (waaronder TMPRSS2) door geactiveerde neutrofielen kan een overdreven activering van bloedplaatjes, coagulatie en trombose en endotheelbeschadiging veroorzaken

Eosinofielen, mastcellen en macrofagen zouden in staat zijn NET's af te geven. Opmerkelijk is dat neutrofielen geprogrammeerde celdood ondergaan die moet worden onderscheiden van apoptose en necrotische celdood (Regulation of Innate Immune Responses by Platelets, Frontiers in immunology, 2019; 10: 1320). Ongecontroleerde NET-vorming draagt ​​bij aan arteriële en veneuze trombose (Neutrophil Extracellular Traps: Villains and targets in arterial, venous and cancer-associated thrombosis, Arteriosclerosis, Thrombosis and Vascular Biology September 2019, Vol. 39, Issue 9). 4. Dynamica van (pro) trombine en fibrine
4.1 Ontstekingsgevolgen van trombine bevorderende microvasculaire trombose, DIC en MOF
Verspreide intravasculaire coagulatie (DIC) en diepe veneuze trombose (DVT) worden expliciet genoemd in een review uit 1999 over het mechanisme tussen infectieziekten en stollingsstoornissen
(Review: Infectious Diseases and Coagulation Disorders, The Journal of Infectious Diseases, 1 July 1999). Het is bekend dat microvasculaire trombi zich vormen na de omzetting van fibrinogeen in fibrine (Pathogenesis of disseminated intravascular coagulation in sepsis, JAMA, 1993 vol. 270). Microvasculaire trombose, falen van meerdere organen en bloeding treden op als gevolg van de consumptie van stollingsfactoren en activering van het fibrinolytische systeem. Hoewel DIC geassocieerd is met zowel de consumptie van bloedplaatjes als de stollingsfactor, zijn hemolytisch uremisch syndroom (HUS) en trombotische trombocytopenische purpura (TTP) niet geassocieerd met de consumptie van stollingsfactoren; HUS en TTP worden gekenmerkt door trombocytopenie (Par. 2.1 beschrijft het mechanisme van VWF en ADAMTS-13 onderliggende TTP).

De functie van protrombine is om de stolling te verbeteren door bloedplaatjes te activeren en door fibrinogeen om te zetten in fibrine. Hoewel trombine een noodzakelijk enzym is, draagt ​​trombine ook bij aan verdere ontsteking. De beheersing van trombines door antitrombine III, weefselfactorroute-remmer en proteïne C-systeem wordt aangetast door infecties zoals SARS-CoV-2, die microthrombose, DIC en multisysteem-orgaanfalen bevorderen. 4.2 Premature fibrinolyse ontregelt de barrièrefunctie van fibrine, resulterend in ophoping van leukocyten en neutrofielen geassocieerd met ischemie
Trombine-activering van endotheel- en immuun-effectorcellen induceert de aanmaak van groeifactoren, chemokines en cytokines en verandert de adhesie. Trombinestimulatie van endotheelcellen resulteert in de expressie van chemokines, waaronder IL-6, IL-8, plaatjesactiverende factor (PAT) en MCP-1, monocyten chemoattractant proteïne, proangiogene mediatoren (groeifactor-bèta), proadhesieve factoren zoals ICAM- 1, een intercellulair adhesiemolecuul en P-selectine. PPACK-alfa-trombine verbetert de rekrutering van leukocyten naar beschadigde endotheelplaatsen.

De binding van trombine aan bloedplaatjes GPIbα vermindert de activering van bloedplaatjes en vroege migratie van leukocyten. Fibrine bindt alfa-trombine en werkt als een fysieke barrière tegen leukocytenmigratie. Na fibrinolyse is de migratie van leukocyten naar de plaats van verwonding uitgebreid, wat suggereert dat fibrine de handel in leukocyten vertraagt. Het induceren van fibrinolyse door rt-PA (Plasminogen Activator) ontregelt de fysieke barrière-activiteit van fibrine, wat resulteert in verbeterde leukocytenmigratie en accumulatie van neutrofielen, geassocieerd met ischemie. De bevinding dat door plasminogeen-activator geïnduceerde fibrinolyse trombo-ontsteking veroorzaakt door ontregeling van de fysieke barrièrefunctie van fibrine, vormt een therapeutisch doelwit

4.3 Urokinase-route: de rol van overexpressie van Serpine1 bij de accumulatie en ontsteking van fibrine
Patiënten met SARS hebben een significant lager aantal bloedplaatjes en lymfocyten

De functie van het urokinase-systeem is het reguleren van fibrinolytische en procoagulatieve reacties om bloeding en vasculaire permeabiliteit te voorkomen. Een studie uit 2013 meldt dat, na een SARS-CoV-1-infectie, overtollig fibrine waarschijnlijk werd gemedieerd door Serpine1-gestuurde remming van de urokinase- en weefseltype plasminogeenactivatoren (PLAU en PLAT) en door de plasmamineactiviteit te blokkeren door α2-plasmine-remmer. SARS ontregelt de profibrinolytische signalering van het urokinase-systeem en verhoogt de expressie van Serpine1 (ook: PAI-1 of Plasminogen activator-1). Fibrine-accumulatie stimuleert profibrotische groeifactoren en cytokines. Collageenafzetting en fibrose zijn het gevolg van fibroblast. Afbraakproducten van fibrine en fibrine verbeteren de vasculaire permeabiliteit, stimuleren de migratie van ontstekingscellen en rekruteren neutrofielen naar de longen.

PLAT dient als antiklontermiddel
Weefselplasminogeen-activator (PLAT of tPA), geremd door Serpine, dient als antiklontermiddel door de splitsing van plasminogeen in plasmine te bevorderen en de afbraak van fibrinestolsels te stimuleren. Dit verklaart waarom een ​​gebrek aan Serpine1 leidt tot bloeding (bloeding). Serpine1-knockout-muizen bezwijken sneller aan SARS-CoV-infectie dan controlegroepen, terwijl de virale belasting niet wordt beïnvloed door Serpine1

Een stollingsprobleem als gevolg van overexpressie van Serpine1
Bij een juiste werking is het urokinase / coagulatiesysteem uitgebalanceerd: bij detectie van schade aan het endotheel veroorzaken cellen de afgifte van fibrine op de plaats van verwonding. Tijdens deze fase waarin het lichaam zijn weefsel moet herstellen, voorkomt Serpine1 de voortijdige afbraak van fibrine. Later in het proces moet fibrine worden opgelost. 4.4. Diffuse alveolaire schade (DAD)

Diffuse alveolaire schade (DAD) is waargenomen als een kenmerkend kenmerk in ernstige gevallen van SARS-CoV-2 (
Pulmonary Fibrosis and COVID-19: the potential role for antifibrotic therapy, The Lancet Respiratory Medicine, 15 May 2020). Naast DAD wordt de aanwezigheid van microthrombi in longslagaders gerapporteerd (Thromboembolic Findings in COVID-19 Autopsies: Pulmonary Thrombosis or Embolism?, Annals of Medicine, 15 May 2020). Acute-fase DAD wordt gekenmerkt door hyaline membranen in de longblaasjes. Bij SARS-CoV-1-patiënten werden exsudatieve-fase DAD en verhoogde macrofagen, samen met oedeem, bloeding en hyaliene membraanvorming waargenomen tijdens het vroege stadium van infectie. 10 dagen na infectie vertoonden SARS-CoV-1-patiënten DAD die tot 100% van de long in beslag namen, evenals longfibrose, wat resulteerde in langdurig consistent verlies van longelasticiteit. Bloeding duidt op voortijdige afbraak van fibrineproducten, aangegeven door vasculaire lekkage in alveolaire ruimtes en ontwikkeling van DAD. 4.5 Pulmonaire fibrose-mechanismen
Vermindering van STAT1, een belangrijk eiwit in door interferon gemedieerde immuniteitsreacties, zorgt ervoor dat SARS-CoV een aangeboren inflammatoire cascade induceert, waaronder grote hoeveelheden macrofagen, neutrofielen en eosinofielen (witte bloedcellen). Na detectie van vocht, bloedingen en fibrine in de longblaasjes, verhoogt een stollingscascade de afgifte van factoren, waaronder F10 die protrombine in trombine splitst (Blood clotting Factor 10. Thrombin activates fibrinogen to fibrin (The coagulation factors fibrinogen, thrombin and Factor XII in inflammatory disorders, Frontiers in Immunology, 2018:9:1731). De ophoping van bloedstolsels zet aan tot fibrinolyse, een systeem om fibrinevormingen te verwijderen door plasmines in plasminogenen te splitsen. 5. Zeldzame gevallen van trombose: antifosfolipide-antilichamen bij patiënten met COVID-19
Een casestudy van drie patiënten die op de IC zijn opgenomen, vermeldt de aanwezigheid van anticardiolipine IgA-antilichamen en anti-β2-glycoproteïne I IgA- en IgB-antilichamen. Deze antifosfolipide-antilichamen zijn gericht tegen fosfolipide-eiwitten, die zelden tot trombose leiden
(Coagulopathy and antiphospholipidantibodies in patients with COVID-19, NEJM, 2020;382:e38). Desondanks zijn de zeldzame gevallen het vermelden waard, omdat rekening gehouden moet worden met een afwijkend ziektebeeld van dat van de meerderheid van de patiëntpopulatie. 

6. Behandeling met LMWH - een beleid voor heparine met laag moleculair gewicht bij hypercoagulante patiënten
Behandeling met anticoagulantia met LMWH (heparine) wordt aanbevolen in het vroege stadium van de ziekte

Bij patiënten met een duidelijk verhoogde D-dimeerwaarde, wordt aanpassing van LMWH geassocieerd met een lagere mortaliteit van 28 dagen (Anticoagulant treatment is associated with decreased mortality risk in severe coronavirus disease 2019 patients with coagulopathy, Journal of Thrombosis and Haemostasis, 27 March 2020). De richtlijnen van de International Society on Thrombosis and Haemostasis schrijven voor dat alle patiënten die opgenomen worden in het ziekenhuis, LMWH toegediend zouden moeten krijgen (Anticoagulant treatment is associated with decreased mortality risk in severe coronavirus disease 2019 patients with coagulopathy, Journal of Thrombosis and Haemostasis, 27 March 2020).

Het risico op door heparine veroorzaakte trombocytopenie (HIT) is een complicerende factor. Trombose-gerelateerde trombocytopenie moet echter worden onderscheiden van HIT. Trombose en trombocytopenie zijn niet paradoxaal: wat wordt waargenomen is VTE met consumptie van bloedplaatjes vroeg in de loop van de ziekte, vóór toediening van heparine. Zelfs bij patiënten met HIT die anticoagulantia nodig hebben, worden lepirudine en argatroban beschouwd als veilige trombineremmers

Volgende deel in deze reeks: interacties van MMP's, met name MMP9, ADAMTS-13 en VWF in ernstige SARS-CoV-2-gevallen
In deze "verslechteringsreeks" heb ik de betrokkenheid van Von Willebrand Factor-plaatjesbinding aan het beschadigde endothelium kort besproken, evenals de rol van ADAMTS-13 bij het temmen van de afgifte van overactieve ULVWF in het plasma. Ontregelde MMP's leveren een opmerkelijke bijdrage aan longfibrose. In het volgende bericht zal ik ingaan op de invloed van overexpressie van MMP-9 op de vorming van trombi en de rol van ADAMTS-13 bij trombose. Hoewel een gebrek aan ADAMTS-13 op zichzelf geen TTP of ischemische beroerte vormt, veroorzaakt een tekort aan ADAMTS-13 een protrombotische toestand die wordt versterkt door andere metalloproteasen en inflammatoire cytokines.

SARS-CoV-2: is aanpassing van ACE2 binnen het RAAS een therapeutische optie?

Dit bericht is de Nederlandse versie van mijn bericht van 18 april 2020: "Pathways to deterioration in SARS-CoV-2: is enhancement of ACE2 in the RAAS system key? 
Het is bekend dat de spike-glycoproteïne (S) van SARS-CoV-2 humaan Angiotensineconverterend enzym 2-receptoren (ACE2-receptoren) gebruikt om de cellen van de gastheer binnen te dringen. De ACE2-receptor wordt aangetroffen in type II alveolaire cellen (AT2) van de longen, slokdarmepitheelcellen, enterocyten uit ileum en colon, neusholte, vasculaire endothelia, nierweefsel, epithelia van de dunne darm, testes en op de epitheelcellen van mondslijmvlies (High expression of ACE2 receptor of 2019-nCoV on the epithelial cells of oral mucosa, International Journal of Oral Science, 24 February 2020 ), evenals in de hersenstam en neurale cortex. SARS valt ACE2-verrijkte cellen binnen, alleen om zichzelf zeer snel te repliceren om andere cellen te samentrekken. Van de infectie van type II-pneumocyten en het vrijkomen van virussen in de luchtwegen met de nabijheid van het pulmonale capillaire bed wordt sterk vermoed dat het systemische verspreiding van het virus naar andere organen mogelijk maakt (ACE2 Receptor Expression and SARS Infection Depend on Differentiation of Human Airway Epithelia, Journal of Virology, December 2005).

Is ARDS een adequate classificatie?
Het is nog onduidelijk wat precies de plotse achteruitgang is bij ernstig zieke SARS-CoV-2-patiënten. Een plotselinge verslechtering bij patiënten met zuurstoftekort wordt aangeduid als "Acute Respiratory Distress Syndrome" (ARDS). Het valt te betwijfelen of ARDS, gekenmerkt door bilaterale infiltraten, hypoxemie en kortademigheid (
Acute Lung Injury and ARDS: Pathophysiology and Treatment, Missouri Medicine, Jul-Aug 2010), een geschikte classificatie is voor de pathofysiologie geassocieerd met SARS-CoV-2 (COVID-19 Pneumonia: ARDS or not?, Critical Care 24, Article number 154 (2020)). Over het algemeen wordt de rol van specifieke pathogenen voor de ontwikkeling van ARDS als moeilijk te beoordelen beschouwd (Acute Respiratory Distress Syndrome and Pneumonia: A Comprehensive Review of Clinical Data, Clinical Infectious Diseases, Vol. 43 Issue 6, 15 September 2006.

Pathologische bevindingen van een vroege COVID-casestudy: overactivering van T-cellen
In één geval met betrekking tot pathologische bevindingen van COVID-19 geassocieerd met ARDS, bleef de zuurstofverzadiging boven de 95% tot de veertiende ziektedag. Op dag 14 van de ziekte verslechterde de hypoxemie en daalde de zuurstofsaturatie tot onder de 60%, gevolgd door een hartstilstand. Bilaterale diffuse alveolaire schade met cellulaire fibromyxoïde exsudaten (wondvocht) werd waargenomen, evenals een toename van pneumocyten en hyaline membraanvorming in de rechterlong; de linkerlong vertoonde longoedeem met hyalinevorming. Deze bevindingen werden beschouwd als suggestief voor ARDS met vroege aanvang. In beide longen werden mononucleaire inflammatoire infiltraten waargenomen die gedomineerd werden door lymfocyten. 


Een van de belangrijkste bijdragen tot verslechtering in dit geval was waarschijnlijk een overactivering van T-cellen. Hoewel perifere CD4- en CD8-T-cellen waren verminderd, waren ze hyperactief. Grote hoeveelheden HLA-DR (die antigeen presenteren en een ligand aan T-cellen leveren) en CD38 (glycoproteïne op het oppervlak van immuuncellen) werden gezien. Concentraties van CCR6 + Th17 (T-helpercellen) waren hoog, CD8 T-cellen bleken hoge concentraties cytotoxische granulaten te bevatten (Pathological findings of COVID-19 associated with ARDS, The Lancet, February 18 2020). Soortgelijke observaties werden gedaan in een onderzoek uit 2005 (ACE2 Receptor Expression and SARS Infection Depend on Differentiation of Human Airway Epithelia, Journal of Virology, December 2005).

Cytokine storm en invasie van het centrale zenuwstelsel en een rol voor Tocilizumab
Cytokine-storm, een overreactie van het immuunsysteem van de gastheren, is geopperd om de mechanismen achter plotselinge verslechtering te verklaren (
The cytokine release syndrome of severe COVID-19 and IL-6R antagonist Tocilizumab may be the key, International Journal of Antimicrobial Agents, 29 March 2020; zie ook Cytokine release syndrome in severe COVID-19, AAAS, 17 April 2020); zie ook Cytokine release syndrome bij ernstige COVID-19, AAAS, 17 april 2020). Een andere route zou de invasie van het centrale zenuwstelsel en de hersenstam kunnen zijn, wat het zuurstofgebrek van patiënten zou kunnen verklaren. Beide routes zijn onlangs door onderzoekers afgewezen, omdat de verstrekte gegevens nog steeds onvoldoende zijn om te bewijzen dat ofwel cytokinestorm ofwel betrokkenheid van het CZS een belangrijke rol speelt bij zuurstofgebrek - naast meervoudig orgaanfalen.

Multi-factor beoordeling
Allereerst moet worden opgemerkt dat er geen homogeen soort ARDS bestaat. ARDS is een definitie die het acute begin van hypoxemie beschrijft (
ARDS subphenotypes: Understanding a heterogeneous syndrome, Critical Care, 24 March 2020). Evenzo is de immuniteitsrespons bij SARS-CoV-2-patiënten heterogeen: behandeling van immuniteitsrespons is geen 'one size fits all'-zaak. Dit wordt nog verder bemoeilijkt door het feit dat immunomodulatie de antivirale activiteit in de gastheer niet mag schaden.

Wat moet en kan worden geleerd van SARS-1 (2003) als het gaat om het beoordelen van medische factoren?

Een uitgebreide beoordeling met meerdere factoren kan nuttig zijn om de routes van SARS, van invasie tot achteruitgang, te verklaren. Het lichaam is een systeem waarin de aangetaste delen elk bijdragen aan de ernst van de ziekte. Immuniteit en cardiovasculaire mechanismen zijn met elkaar verweven, daarom kan niet worden aangenomen dat deze mechanismen onafhankelijk bijdragen aan verslechtering. Deze systemen zijn een basisbehoefte om de machine draaiende te houden. Ik zou zeggen dat SARS in hoofdzaak een onbalans in de homeostase veroorzaakt, gekenmerkt door ACE2-verstoring en onbalans van het Renine-Angiotensine-Aldosteronsysteem, schade aan endotheelweefsel, hetgeen aanzet tot de ontstekingssoep / cascade, trombotische gebeurtenissen - deze mechanismen versterken elkaar in een overdrive-modus. Het samenspel tussen ontsteking en veneuze trombo-embolie is niet alleen een oorzaak en gevolg, maar een verweven interactie. Zie bijvoorbeeld: '

Wat SARS-CoV-1 duidelijk heeft gemaakt, is dat stollingsstoornissen vaak voorkomen in ernstige gevallen. Endotheliumbeschadiging kan een vroege oorzaak zijn van een ontstekingscascade, gevolgd door trombotische voorvallen. In een SARS-recensie uit 2004 werd gezegd dat de SARS-epidemie-ervaring van 2003 een verhoogde waakzaamheid tegen beroerte en andere trombotische SARS-gerelateerde gebeurtenissen zou moeten rechtvaardigen bij toekomstige uitbraken van coronavirussen (Large artery ischaemic stroke in SARS, Journal of Neurology 251, October 2004).

Ik kom bij wat me intrigeerde in het kader van andere ziektestudies (cardiovasculair) en wat mijn aandacht weer trok tijdens de vroege fase van de SARS-CoV-2-pandemie: dat is de plausibele rol van ACE2 als onderdeel van het RAAS-systeem en het nadelige verlies van ACE2 tijdens infectie met SARS-CoV-2


Het RAAS-systeem en ACE
Het renine-angiotensine-aldosteronsysteem (RAAS) reguleert de bloeddruk en de vochtbalans. De RAS genereert angiotensine II (Ang II), dat zich bindt aan receptoren in de hersenen, nieren, vasculatuur en immuunsysteem. Angiotensinogen (Agt) is een substraat van renine. Renin splitst Agt op Angiotensin I (Ang I), en wordt vervolgens door ACE gesplitst op Ang II. Renine komt voornamelijk tot uiting in de nieren. Mastcellen zijn betrokken bij de afgifte van renine. In 2006 werd gevonden dat de afgifte van renine door cardiale mastcellen kan worden veroorzaakt door ischemie (
Classical Renin-Angiotensin System in Kidney Physiology, Comprehensive Physiology, Vol. 4 Issue 3, July 2014)).

Bradykinine is een substraat voor ACE. Bradykinie  heeft vaatverwijdende en natriuretische eigenschappen. ACE inactiveert bradykinine en staat daarom bekend als kininase II. ACE-remmers verhogen het niveau van bradykinine (
Unraveling the pivotal role of Bradykinin in ACE inhibitor activity, American Journal of Cardiovascular Drugs, 3 June 2016). De remming van ACE wordt geassocieerd met angio-oedeem (Effect of bradykinin receptor antagonism on ACE inhibitor-associated angioedema, Journal of Allergy and Clinical Immunology, July 2017, Vol. 140 Issue 1).

ACE-AngII moet in evenwicht zijn met ACE2-Ang 1-7

ACE2 splitst een residu van Ang I om Ang 1-9 te vormen en zet Ang II om in vaatverwijdend Ang 1-7 (NCBI: gene ID), dat Ang II blokkeert en ACE remt (
(ACE2, a new regulator of the renin-angiotensin system, Trends in Endocrinology and Metabolism, May 2004). In een studie uit 2008 werd voorgesteld dat verlies van ACE2-expressie en lokaal verhoogde Ang II-productie lekkage van longvaten veroorzaakte na SARS-infectie. Penninger noemt de katalytische inactivering van metabolieten van bradykinine door ACE2 als risicofactor (The discovery of angiotensin-converting enzyme 2 and its role in acute lung injury in mice, Experimental Physiology, 25 April 2008). Vermindering van ACE2 tijdens infectie met SARS is waarschijnlijk de oorzaak van onevenredige niveaus van bradykinine, wat tot lekkage kan leiden.

Ang II staat erom bekend een centrale rol te spelen bij endotheeldisfunctie. Ang II verhoogt niet alleen de bloeddruk via vasoconstrictie (vernauwing van bloedvaten), de werking op de Ang II type I-receptor (AT1) heeft een negatieve invloed op de vaatwand en versterkt oxidatieve stress, wat resulteert in endotheelbeschadiging en endotheelcelapoptose. Oxidatieve stress verhoogt de expressie van plasminogeenactivatorinhibitor type I (PAI-1), wat resulteert in de rekrutering en binding van ontstekingscellen aan het endotheel weefsel, wat leidt tot ontsteking en trombose (A review of the role of bradykinin and nitric oxide in the cardioprotective action of Angiotensin-Converting Enzyme Inhibitors: Focus on Perindopril, Cardiology and Therapy 8, 1 October 2019).

Verbetering van ACE2 kan de sleutel zijn (
New agents modulating the renin-angiotensin-aldosterone system- Will there be a new therapeutic option?, Experimental Biology and Medicine, 19 July 2016). Een recente follow-up van de studie van Penninger uit 2008 stelt menselijke recombinant ACE2 voor voor een ander mechanisme dat plausibel lijkt: de studie uit 2020 toont remming van het virus door hrsACE2 (Inhibition of SARS-CoV-2 infections in engineered human tissues using clinical-grade hrsACE2, Cell Journal Pre-Proof, April 2020). Eerder werd een vergelijkbare therapie voorgesteld met recombinant humaan ACE2, rhACE2, om de Ang II-plasmaspiegels te verlagen en Ang 1-7 en 1-5 te verhogen (Recombinant human ACE2: acing out Ang II in ARDS therapy, Critical Care, 13 December 2017) .

In het volgende onderdeel zal ik de prevalentie van trombotische voorvallen en longembolie geassocieerd met SARS-CoV-1 (2003) en SARS-CoV-2 bespreken. Twintig jaar SARS heeft duidelijk gemaakt dat stollingsstoornissen optreden in SARS-gevallen - zelfs patiënten die terloops worden beschouwd als "niet in gevaar" lopen in feite risico op trombotische voorvallen. Houd er rekening mee dat al deze mechanismen, van ACE2-stoornis en ontregeling van het RAAS, ontsteking, trombose en trombocytopenie (stollingsstoornissen) en cytokinestorm met elkaar verweven zijn; deze mechanismen en pathologieën dragen bij tot verslechtering in ernstige gevallen van COVID-19.
