Mechanisms involved in SARS-CoV-2 / COVID-19

It has been clear since the SARS epidemic of 2003 that coronavirus diseases caused by SARS-CoV infections are thromboinflammatory diseases. As of March 2020, when SARS-CoV-2 was politically (finally) declared a pandemic, COVID-19 was still treated like a main pulmonary disease. Reseachers have been pointing out that the underlying mechanisms are characterized by hyperinflammation, hypercoagulation, complement cascades and dysregulation of the Renin-Angiotensin-Aldosterone System, ultimately leading to thromboembolic events in multiple organs (MOF), stroke and post-COVID systemic diseases, such as Guillain-Barré and cardiomyopathy. 

To provide a view into the complexity of COVID-19, I share with you the most prominent research papers on the mechanisms and pathogenicity of SARS-CoV-2. Some of the research papers of the biggest relevance for COVID-19 complications are as old as 60 years. This is why the paradigm should be that "Lots of the mechanisms causing COVID-19 are well-known, but we have to find the best practices to treat its complications".

I only refer to verified and reviewed research and scientific viewpoint papers citing verified references. If paper has already been peer reviewed but still awaitens the editory process (early release), I will mention this. Extensive reading recommendations on SARS-CoV-2/COVID: all mechanisms involved (Part 1)

I this message, I will present you all the research papers on Coronavirus Disease 2019 (COVID) I have bookmarked since January 2020. These are extensive papers on SARS-CoV-1 (2003) and SARS-CoV-2 (2019) I have retrieved from the PubMeb database on NCBI and the Wiley Library.
 
All mechanisms involved in COVID-19

There is a ton of information on the mechanisms underlying SARS-CoV infection and the severity of COVID-19. This is the case because (a) SARS-CoV-2 is not entirely new, but shares great similarities with SARS-CoV-1 (2003) and (b) because not all mechanisms are exclusive to SARS' etiology, but are also seen in infections caused by other pathogens, ór, even common in systemic syndromes (say, for example, aHUS). Scientists do compare diseases and viruses, to reveal patterns in similarities and differences. Rarities can reveal a lot about mechanisms underlying other diseases. In short, thrombosis in pregnancy caused by a deficiency in ADAMTS-13, which leads to failure to cleave Von Willebrand Factor (VWF) and therefore contributes to extensive thrombus formation, is also seen in some severe (juvenile) cases of COVID.

Thus, systemic syndromes, common immunity and thromboinflammatory mechanisms need an in-depth view to find solutions to balance the key factors in COVID-19.

Corona en het complementsysteem

Onderliggende mechanismen van de complicaties van SARS-Coronavirussen (2003-2020)
In deze serie ga ik in op de mechanismen die bijdragen aan de ernst van COVID (en het vorige SARS-Coronavirus, SARS-CoV-1 uit 2003). Eerder besprak ik de rol van het RAAS-, trombose en schade aan het centraal zenuwstelsel in gevallen van SARS-CoV-2. Het immuunsysteem, ontstekingsmechanismen en het complementsysteem zijn sterk met elkaar verweven mechanismen. De rol van het complementsysteem is het op gang brengen van een immuniteits- en onstekingsreactie. Zoals ik in dit bericht zal bespreken, zorgt dit mechanisme voor schade aan het endotheel weefsel en obstructie van de bloedvaten.


1 Het complementsysteem: een belangrijke maar 'explosieve' schakel in de immuniteitsreactie
Het complementsysteem verwijst naar de activering van het adaptieve immuunsysteem van de gastheer in reactie op pathogenen, zoals virussen. Een belangrijke functie is lyse door het groepje complementeiwitten C56-9 (C5 tot 9 = C5bC6C7 tot C8 tot C9) dat met "Membrane Attack Complex" (MAC) wordt aangeduid. Een MAC maakt gaten in de ziekteverwekker ('lysing'), om de ziekteverwekker onschadelijk te maken. Opsonisatie is het proces waarbij antilichamen zich binden aan de ziekteverwekker. Fagocyten (witte bloedcellen) sluiten ziekteverwekkers in hun kern in, om zo een isolerend laagje te vormen. Het gezonde weefsel wordt op deze wijze beschermd tegen de ziekteverwekker. Het complementsysteem genereert ook anafylatoxinen. Anafylatoxinen die een heftige ontstekingsreactie op gang brengen, zijn C5a en C3a. Deze complementfactoren zorgen voor een 'explosieve' reactie op infecties. 

 
Het is een normale reactie van het lichaam op ziekteverwekkers om een complementcascade, ofwel een waterval van complementfactoren, op te wekken en zo het immuunsysteem op gang te brengen. De complementcascade kan echter nadelig uitpakken als deze overdreven actief is. In het geval van COVID reageert het complementsysteem hyperactief op de infectie met het virus en raken de gezonde weefsels beschadigd. Specifiek zijn de complementfactoren C5a en C5b-9 (de MAC) betrokken bij beschadiging van de gezonde weefsels van de geïnfecteerde. 

2 Hoe het complementsysteem bijdraagt aan de ernstige complicaties van COVID-19

2.1 Door het complementsysteem geïnduceerde acute longschade (ALI) en ARDS (Acute ademhalingsstoornis
Door het genereren van anafylatoxinen C3a en C5a, de vorming van de amplificatielus waarbij een grote hoeveelheid complementfactoren vrijkomt en de vorming van de MAC via C3- en C5-convertases, ontstaat immuungerelateerde schade. C5a wekt een ontstekingsreactie op door neutrofielen, monocyten, eosinofielen, fagocytische cellen, op granulaat gebaseerde enzymen en T-lymfocyten aan te trekken. 

Wat typerend is voor SARS-Coronavirussen, is een snelle progressie naar ARDS. Zoals het geval is met vogelgriepvirussen, worden veranderingen in het longweefsel 11 dagen na het begin van de symptomen waargenomen, met een snelle ontwikkeling van acuut longletsel (ALI). Verhoogde niveaus van C5a worden gevonden in het vocht in de longblaasjes (BALF). Dit is niet het geval bij seizoensgebonden influenza A (IVA), ofwel de A-variant van het griepvirus. SARS-CoV-1 en SARS-CoV-2 zijn totaal verschillend van griepvirussen. Niet alleen omdat ze geen voorouder delen met griepvirussen, maar ook omdat acuut longletsel geassocieerd met overmatige C5a-activering typerend is voor SARS-Coronavirussen, MERS-Coronavirussen en vogelgriep.

2.2 Aanvulling op anafylatoxine C5a en IL-8 inductie van reactieve zuurstofspecies (ROS)

Zowel C5a als interleukine IL-8 wordt gesynthetiseerd door longepitheelcellen, macrofagen, endotheelcellen en neutrofielen. C5a is in staat om IL-8 te versterken om de longdysfunctie te verhogen. Na activering van neutrofielen en monocyten door C5a, wordt een oxidatieve uitbarsting gegenereerd, gevolgd door het vrijkomen van Reactive Oxygen Species (ROS). Grote hoeveelheden ROS worden waargenomen in fibrotische longen.


2.3 Wisselwerking tussen het complementsysteem en de NETs

C5a is in combinatie met granulocyt-macrofaag kolonie-stimulerende factor (GM CSF) in staat om de afgifte van NET's (neutrofiele extracellulaire vallen) te induceren en om macrofagen en endotheelcellen te activeren om vaatlekkage en de afgifte van NETs te bevorderen. NETs verhogen de doorlaatbaarheid van de longvaatjes. NETs en hypoxie dragen verder bij aan de ontsteking van het endotheel weefsel. Terwijl component-opsonisatie NETs bevordert, remt blokkade van complementreceptoren CR1 en CR3 NETosis (geprogrammeerde celdood). Neutrofielen en NETs bevatten C3, Factor B en Factor P (Properdin), die essentieel zijn voor het genereren en stabiliseren van C3 en de complementcascade. NETs activeren het stollingsproces. 


3 Complementgerelateerde schade bij SARS-CoV-2-patiënten

Afzettingen van MAC (C5b-9), C4d en MASP-2 werden waargenomen in het vaatstelsel bij SARS-CoV-2-patiënten. Patiënt 1 vertoonde ernstige hemorragische pneumonitis (longontsteking met bloedingen) met afzetting van fibrine in het vaatweefsel, celnecrose en trombotisch nectrotiserend letsel-syndroom. Naast fibrine werd C3d in het vaatweefsel aangetroffen. Uitgebreide C4d- en MAC-afzetting werd waargenomen in het vaatweefsel van de longblaasjes. MAC-afzetting werd ook gezien in normaal lijkende huidvaatjes. Patiënt 2 vertoonde een vergelijkbaar letsel, hemorragische pneumonitis, afzetting van fibrine en dominante afzetting van MAC-formaties in het fijne vaatweefsel. Opmerkelijk is dat MAC-afzettingen werden gevonden in normaal ogend longweefsel, terwijl C4d alleen werd gevonden in de beschadigde vaten. Patiënt 1 en 2 vertoonden geen diffuse alveolaire schade (DAD); er werden geen hyalinemembranen aangetroffen.

Patiënt 3 had blauwpaarse billen, trombogene vaatonsteking met necrose, afbraak van witte bloedcellen en uitgebreide MAC-afzettingen in het vaatweefsel. Bij patiënt 4 werden paarse plekken gezien op voetzolen en handpalmen. Occlusieve trombi werden gevonden in een slagader, inclusief uitgebreide afzettingen van MAC-formaties, C3d en C4d. Op CT werden herseninfarcten gezien. Net als patiënt 3 en 4 vertoonde patiënt 5 blauwpaarse plekken over het lichaam. Lymfocytische infiltraten en trombi gingen gepaard met MAC en C4d

4 Farmaceutische interventie gericht tegen de complementcascade in COVID-19
Therapeutische opties voor remming van de complementcascade zijn:

FAQ Corona

Is dit virus een nieuw virus, zoals zo vaak wordt gezegd?
Nee. Dit virus heet voluit SARS-CoV-2 en is qua ziekteverschijnselen bijna geheel identiek aan SARS-CoV-1, het coronavirus uit 2003. Ten opzichte van 2003 is dit coronavirus sterker in de succesvolle verspreiding. De klachten op lange termijn, zoals schade aan de longen, nieren, het zenuwstelsel en (auto-)immuunziekten, zijn hetzelfde als die van 2003. Ook de voor SARS typerende verschijnselen op korte termijn, zoals trombose-verschijnselen, zijn sinds 2003 bekend. Als een wetenschapper níet politiek wordt beïnvloed en tóch zegt dat dit virus nieuw en onbekend is, dan heeft hij maar liefst 20 jaar in de slaapstand geleefd.

Waarom zou ik me laten testen als ik géén klachten heb?
Mensen verspreiden het virus al op het moment dat zij geen klachten hebben. De periode waarin iemand is geïnfecteerd, maar nog geen klachten heeft, heet de incubatietijd. In de incubatietijd is de virale lading hoog. Voor corona geldt zelfs dat de virale lading in de keelneusholte het hoogst is voordat de symptomen optreden. Door te praten en uit te ademen, kan een persoon die nog geen klachten heeft (zoals hoesten, koorts of hoofdpijn) het virus verspreiden.

Waarom is "blijf thuis als u klachten hebt" niet juist?
Het motto "Blijf thuis als u klachten hebt" is niet juist, omdat pre-symptomatische transmissie van het coronavirus plaatsvindt. Vroegtesten/presymptomatisch testen in de incubatietijd geeft duidelijkheid. Hoewel niemand van vroegtijdig testen moet worden uitgesloten, is vroegtesten van zwaarwegend belang voor mensen in contactberoepen en cliënten/kinderen/patiënten. Mensen in de zorg en onderwijspersoneel hebben contact met grote groepen mensen en (indirect) met hun familieleden. Juist in de zorg- en onderwijssector kunnen clusters van virusbesmettingen ontstaan. Het is dus zaak om besmettingen zo vroeg mogelijk te detecteren, nog vóórdat het contact met de cliëntgroep/klas/collegezaal/afdeling plaatsvindt.

Waarom is de 1,5 meter-afstandsregel niet voldoende?
Gesteld dat de zwaarste, grotere druppels een beperkte afstand afleggen (het onderscheid is in de natuurkundige werkelijkheid niet zo strikt), dan is deze afstand minimaal 3 meter. 
In een gesloten ruimte stapelen de virusdeeltjes zich op. De lucht raakt verzadigd met de uitgeademde lucht van de aanwezige personen. Het houden van afstand is niet voldoende om verspreiding via inademing van deze lucht tegen te gaan. Virusdeeltjes in uitgeademde lucht kunnen zich zelfs over 8 meter afstand verplaatsen.

Afstandsregels helpen niet tegen aërogene verspreiding van het coronavirus. Aërosolen zijn kleine deeltjes die zich in een 'turbulente' gaswolk verplaatsen en zich opstapelen in een gesloten ruimte. Tegen aërogene transmissie, een onderwerp waarin TNO is gespecialiseerd en waar TNO adviezen over uitgeeft, helpen goede ventilatie (24 uur per dag!), het laten draaien van mechanische ventilatie op 100% buitenlucht, mechanische ventilatie met een HEPA-filter en het dragen van mondkapjes. UV-C is de meest effectieve straling uit het UV-spectrum om het virus te destabiliseren. Om de lucht niet te snel te laten vervuilen, moeten ook niet veel mensen in een gesloten ruimte verblijven. 

Is het mondkapje een vervangende maatregel voor plaatsen waar men geen afstand kan houden?
Nee. Het mondkapje is een belangrijke maatregel die verspreiding van het virus direct tegen moet gaan. Het mondkapje is géén vervanger voor de afstandsregel. Het houden van afstand helpt namelijk alleen tegen directe druppels, niet tegen virusdragende aërosolen. Zoals bij vorige vraag aangestipt, helpt afstand houden niet tegen de opstapeling van virusdeeltjes in een gesloten ruimte.

Het mondkapje is ook géén middel om een signaal aan anderen af te geven. Het mondkapje is een essentieel middel om aërogene transmissie (de prominente verspreidingsroute van het coronavirus) tegen te gaan. Het is dus géén vervanger of middel met een signaalfunctie.

Wat is er waar van de berichtgeving dat mondkapjes schadelijk zouden zijn?
Op sociale mediaplatforms, zoals Facebook, worden nepberichten geplaatst over de "schadelijkheid van het mondkapje". Deze berichten worden gemaakt door nepaccounts en door oplichters. Recentelijk zijn gestolen foto's en gestolen profielen geïdentificeerd en gerapporteerd. Foto's van mensen met ernstige huidziekten waren gestolen van sites van ziekenhuizen en apothekers. Deze foto's werden verknipt en voorzien van de valse berichtgeving dat de foto's de schadelijkheid van mondkapjes lieten zien.

Nepberichten over vochtophoping en ziekte door stress van een mondmasker worden verspreid door een zogenaamde arts die holistische diëten blijkt te willen verkopen. Deze "arts" claimt dat het holistische dieet, dat bestaat uit dagelijks wandelen en 200 gram groenten eten, voldoende is voor het immuunsysteem. Een bizar financieel motief (het aan de man brengen van het dieet) schuilt dus achter het bericht dat de drager van een mondmasker ziek zou worden door vochtophoping.

Kan een mondkapje wel schadelijk zijn?
Ja, als u iedere dag een gebruikt mondkapje opnieuw gebruikt en als u de hele dag hetzelfde mondkapje draagt. Het herbruikbare mondkapje moet wel worden gedesinfecteerd. Een wegwerpmondkapje kan na het dragen in een afgesloten zak in de container. Het is natuurlijk ook niet de bedoeling om het mondkapje aan te raken. Als u het verwijdert, pak het dan bij de uiteinden van de elastieken. Raak het kapje zelf niet aan. Was uw handen na het verwijderen van beschermingsmaterialen, zoals het mondkapje.

Is het waar dat een mondkapje niets tegenhoudt?
Nee. Dat een mondkapje "niks doet" is een schadelijke volkswijsheid die niet wordt bevestigd door wetenschap. Mensen denken dat virusdeeltje zo door de ruimte zweven en door de vezels van een masker binnenkomen. In werkelijkheid zit een virusdeeltje in een coating van zouten en speeksel. Na verplaatsing door de ruimte slaat het residu neer op een masker. Het zijn geen deeltjes die zo in hun geheel op het masker landen en door de vezels kruipen. Bovendien is er een belangrijk onderscheid tussen maskers.

Voor alle maskers geldt: hoe dichter geweven de stof en hoe meer gelaagd, hoe beter. Het onderscheid tussen professionele maskers is:
1. Maskers van de categorie FFP2 en hoger/KN95: minstens 95% filtercapaciteit, beschermen zowel de drager als de ander;
2. Chirurgische maskers (papieren wegwerpmaskers): beschermen anderen tegen ademhalingsdruppels afkomstig van de drager.

Heeft de overheid gezegd dat alleen niet-werkende mondkapjes mogen worden gedragen?
Nee. Een sarcastische columnist van VK heeft dit citaat verzonnen. Het citaat is in een knipsel verspreid en vervolgens een volkswijsheid geworden. Niemand heeft gezegd dat alleen niet-werkende mondkapjes mogen worden gedragen. Niemand heeft gezegd dat u een boete krijgt als u een gecertificeerd mondkapje draagt. Dat komt allemaal uit de koker van een "grapjas".

Wordt het virus minder ernstig?
Er zijn geen aanwijzingen dat het virus minder ernstig zou zijn. In Singapore is een mutatie van het coronavirus getroffen die "milder" lijkt uit te pakken. Dat geldt niet voor andere landen. De volkswijsheid, dat het virus minder ernstig zou zijn, is wensdenken. Dat wensdenken wordt schadelijk als de aandacht voor het treffen van voorzorgsmaatregelen verslapt! Blijf vooral wél voorzichtig.

Prevalentie en incidentie (epidemiologie)
Ik kan niet genoeg benadrukken dat exponentiële groei en incubatietijd allesbepalend zijn. Dat er nu niet "veel" mensen op de ic liggen, zegt niets over de toestand in de komende weken. Het kan bijvoorbeeld nog een week duren voordat iemand ernstiger ziek wordt en het aantal geïnfecteerden kan explosief stijgen. Daarop moeten mensen anticiperen. Op het moment dat het aantal besmettingen en ic-opnames afzwakt, is het zaak om de teugels niet te laten vieren. Dat is gebeurd en daarom is er nu een grote toename. Helaas zijn er overheidslieden die het verschil tussen "incidentie" en "prevalentie" niet begrijpen. De incidentie is het aantal besmettingen binnen een bepaald tijdsbestek, bijvoorbeeld binnen één week. De prevalentie is het geheel. Op grond van de incidentie wordt door de overheid besloten om géén maatregelen te treffen, terwijl strategieën in het perspectief van de prevalentie moeten worden geplaatst.

Hoe gunstig is Nederland voor een virus? 
In Nederland kan nagenoeg alles en dat is zeer gunstig voor een virus. De vrijheid van de mensen is niet aangetast. Vanaf maart is het druk geweest op de stranden en in winkelstraten en deze zomer konden Nederlanders naar risicogebieden met code oranje afreizen. Na terugkeer wordt niet verplicht getest en mensen die uit hoog risicogebieden zijn teruggekeerd, hoefden niet verplicht in quarantaine. Mondkapjes zijn niet verplicht in gesloten publieke ruimten. Alle ingrediënten voor snelle verspreiding van het virus zijn aanwezig, terwijl het virus niet is afgezwakt. Het wekelijkse geconstateerde aantal besmettingen ligt op 3500. Met een volop rondwarend virus (SARS, het predicaat dat slechts aan zeer ernstige coronavirussen wordt verleend) waarvan sinds 2003 bekend is dat het ernstige schade aan organen toebrengt, is de staat van Nederland belabberd slecht.

Welke misvattingen krijgen teveel aandacht?
Bewust relativerende, dus bagatelliserende opmerkingen over het virus zijn doelloos en kennen geen wetenschappelijke basis. Opmerkingen als "De angst is dodelijker dan het virus", "Door alle aandacht voor corona gaan meer mensen dood aan andere ziekten waarvoor ze nu niet worden behandeld", "In het verkeer vallen meer doden", ""Herstelde" coronapatiënten krijgen hun ernstige klachten niet door het virus, maar door de reactie van hun eigen lichaam op het virus" zijn de bekende clichématige drogredenen. Oorzaak en gevolg worden omgekeerd. 

Bovendien maakt het voor een virus werkelijk geen barst uit wat mensen over het stukje RNA denken. Het virus heeft maar één taak: zich zo snel mogelijk verspreiden. Hoe meer mensen bezig zijn met hun ongenoegen uiten over maatregelen die verspreiding beperken, zoals een mondkapje of het verbieden van reizen naar risicogebieden, hoe succesvoller het virus zich onder de mensen kan verspreiden. Dat mensen de ruimte hebben gekregen om te zeuren over het "eeuwige" verlies van hun vrijheid, heeft Nederland alleen maar op een onnoemelijke achterstand gezet in de aanpak.

Door het virus ongecontroleerd rond te laten waren, kunnen mensen niet veilig naar het ziekenhuis. Als iemand die behandeld moet worden voor een andere ziekte, is aangewezen op het OV of ziekenvervoer, dan kan de afspraak niet veilig worden afgelegd. In het OV wordt niet gehandhaafd en in het ziekenvervoer dragen andere patiënten geen mondmaskers. In de academische ziekenhuizen ontbreken adequate maatregelen. Dít brengt patiënten in gevaar. Daar komt bij dat de zorg op zeer korte termijn weer overspoeld wordt door COVID-patiënten en door het gebrek aan capaciteit is genoodzaakt om de reguliere zorg te beperken. Het komt niet door de "overmatige aandacht" voor het virus dat mensen niet geholpen kunnen worden voor andere ziekten, het komt door het gebrek aan maatregelen om de zorg veilig en met voldoende capaciteit te laten verlopen.

Dat "herstelde" coronapatiënten ernstige klachten krijgen ná besmetting met het virus, heeft ermee te maken dat (1) schade en chronische klachten door het virus worden veroorzaakt (virulentie) en (2) dat immuniteitsgerelateerde ziekten optreden als gevolg van infectie met het virus (systemische aandoeningen). Systemische aandoeningen als SIRS, MIS-C en immuniteitsgerelateerde aandoeningen als Guillain-Barré die het gevolg zijn van COVID, zouden zonder infectie met het coronavirus níet zijn opgetreden. Hetzelfde geldt voor cardiovasculaire schade en hersenbloedingen die het gevolg zijn van COVID. De opmerking binnen deze context dat "Chronische aandoeningen niet het gevolg zijn van corona" is dus niet alleen respectloos, maar ook pertinente onzin. In alle gevallen, of het nou direct gebeurt of na een paar weken of maanden, is het virus de oorzaak. Alle hyperbolen, manke vergelijkingen en andere drogredenen om de ernst van SARS te ontkennen, kunnen direct bij het vuil worden gezet.

Waarom treft de overheid geen maatregelen om het virus in te dammen?
Vanaf het begin, maart 2020, gaf de overheid bewust de brui aan indamming. Met de kennis van toen was duidelijk dat een korte lockdown met testen en isoleren noodzakelijk was om de verspreiding van het virus een halt toe te roepen. Zonder motivering werd alles ingezet op het scenario ongecontroleerde "groepsimmuniteit", terwijl dit scenario geen wetenschappelijk fundament heeft. Met verdraaiing van feiten, verknipte presentaties, onderbuikgevoelens gepresenteerd als "bewijs", is een poging gedaan om de bevolking te overtuigen van de illusie van controle. Voor de doelstelling groepsimmuniteit worden mensen opgeofferd. Mensen zijn in dit scenario slechts nummers. Door iedere keer een discussie te beginnen over luttele zaken, op het niveau van "we weten niet of mondkapjes werken", wordt de bevolking afgeleid van waar het werkelijk om gaat: dat er geen maatregelen zijn getroffen. Drogredenen verzekeren dit land van een onnodige achterstand.

Initiatieven vanuit de bedrijfssector voor het vervaardigen van mondkapjes om de zorgsector te voorzien, waren er al rond maart 2020. Daar is géén gehoor aan gegeven. Private producenten van rt-PCR-tests hebben geen goedkeuring gekregen om in het voorjaar van 2020 op grote schaal testmateriaal aan te leveren. De kansen die werden aangereikt vanuit het bedrijfsleven, zijn niet aangegrepen.

In juni 2020 was duidelijk dat met het openstellen van de mogelijkheid om naar hoog risicogebieden te reizen, om zonder restricties terug te keren en weer aan het openbare leven deel te nemen, een  stijging van het aantal besmettingen zou komen. Op scholen en in verzorgingshuizen zullen wederom clusters van besmettingen ontstaan. Waarom wordt het treffen van adequate maatregelen nagelaten? De verklaringen zijn (1) dat de onhaalbare groepsimmuniteit nog altijd een streven is; (2) dat niet mag worden bijgesteld, omdat het bijstellen van het beleid wordt gezien als het "toegeven" van het falen; (3) dat economische kortetermijneffecten voorrang krijgen in de strategie van de overheid.

Is handhygiëne nog steeds belangrijk?
Ja. Het coronavirus kan worden verspreid via voorwerpen en materialen. Het was al bekend dat corona een vettige envelop heeft die ervoor zorgt dat het virus tot wel 3 dagen stabiel blijft op materialen zoals plastic en karton. Daarnaast blijkt corona een suikerlaagje te hebben om het virus lang sterk te houden. De handen moeten worden gedesinfecteerd of met zeep worden gewassen om het vet- en suikerlaagje van het virus te verwijderen.

Wat moet er nu gebeuren?
- Mensen die terugkeren uit hoog risico-gebieden moeten een quarantaineperiode van 14 dagen aanhouden om te kunnen testen;
- Iedereen moet in gesloten publieke ruimten een mondkapje op;
- Er moet beleid worden uitgevaardigd. Er is sinds maart 2020 géén beleid. De maatregelen die in het beleid moeten worden betrokken, moeten niet worden afgeschaft zodra de exponentiële groei van het aantal infecties afneemt. Stoppen met blussen voordat de brandhaarden zijn bestreden is typerend voor Nederland en daardoor blijft de crisis langdurig in stand;
- Houd minimaal 2 meter afstand;
- Gebouwen moeten adequate ventilatie toepassen. Ventilatie volgens het Bouwbesluit is niet voldoende. Het Bouwbesluit is namelijk niet afgestemd op het tegengaan van aërogene verspreiding van virussen;
- Hercirculatie moet worden uitgeschakeld. In plaats daarvan moet 100% buitenlucht worden aangevoerd. Is dat écht niet mogelijk, dan moet de lucht worden gedesinfecteerd en/of gefilterd met een HEPA-filter;
- Als het gebouw ramen en deuren heeft, zet deze dan open voor natuurlijke ventilatie. Plaats géén mensen in de luchtstroom!;
- Minimaliseer het aantal mensen dat in een lokaal aanwezig is, om vervuiling van de lucht te beperken;
- Gebouwen moeten ook in onbemande staat 24 uur per dag geventileerd worden. Als de mensen uit het gebouw zijn, wordt de lucht geheel ververst. De mechanische ventilatie moet na de vakantie minstens een dag en een nacht draaien;
- Er moet landelijk ventilatiebeleid komen, uitgevaardigd op advies van experts in aërogene transmissie;
- Vroegtesten moet worden ingevoerd om verspreiding in de incubatietijd tegen te gaan. Uiteraard moet de persoon vanaf de aanvraag van de test tot en met de tweede uitslag (2 x testen) thuis verblijven.
